原发性高血压合并2型糖尿病患者脉压和心室功能的关系

目的:探讨原发性高血压(EH)、2型糖尿病(DM)及糖尿病合并高血压(DM+EH)患者血压和脉压与左心功能的关系。方法:对241例患者,高血压75例(EH),2型糖尿病患者(T 2DM)77例和高血压病合并2型糖尿病(EH+T 2DM)89例进行血压检测和超声心动图检查。结果:DM+EH组收缩压、脉压、左心室重量显著大于DM组及EH组(P<0.05),脉压增大的DM+EH组舒张早期血流峰值速度/舒张晚期血流峰值速度(E/A)显著大于DM组及EH组(P<0.05),左心室射血分数3组间无显著性差异(P>0.05);左心室舒张功能异常患者收缩压、脉压均显著高于左心室舒张功能正常患者(P<0.05),3组左心室收缩功能正常无显著性差异(P>0.05)。结论:脉压增大患者易引起心室肥厚及心脏舒张功能受损。

Relationship between pulse pressure and ventricular function in hypertension patients with type 2 diabetes mellitus

Objective:To investigate the characteristics of pulse pressure of hypertension with or without type 2 diabetes mellitus(DM),and their relationship with ventricular function.Methods: All 241 patients were divided into type 2 DM(n=77),hypertension(EH)(n=75),and hypertensive DM(n=89) groups.Echocardiogrm and blood pressure were examined in all patients.Results: Systolic blood pressure(SBP) and pulse pressure(PP) were significantly increased in hypertensive DM compared with type 2 DM or EH,indicating the left ventricular mass(LVM)was larger(P<0.05).The percentage of diastolic dysfunction was significantly low in hypertensive DM compared with type 2 DM or EH(P<0.05),and SBP had marked impact on left ventricular diastolic function.Conclusion: Heart of patients with increased pulse pressure were severely damaged.
