糖皮质激素治疗效果对原发性肾病综合征患者血脂的影响

为了解用糖皮质激素治疗对原发性肾病综合征患者血脂、脂蛋白和载脂蛋白的影响,采用生化方法测定了52例原发性肾病综合征患者使用糖皮质激素治疗前后的上述指标.结果:对糖皮质激素敏感者(完全缓解组)的TC,TG,LDL-c和ApoB100较治疗前明显下降(P<001),ApoA1/ApoB100较治疗前明显增高(P<001).部分敏感者(部分缓解组)的TC,LDL-c和ApoB100也较治疗前有明显下降(P<005和<001).ApoA1/ApoB100较治疗前明显升高(P<001).而对糖皮质激素抵抗者(无效组)的上述指标无明显变化(P>005).各组治疗后各参数之间比较,完全缓解组上述指标下降最显著.结果提示,糖皮质激素治疗效果对原发性肾病综合征患者的血脂、脂蛋白和载脂蛋白有明显影响.对持续性高脂血症患者,除原发病治疗外,应给予积极的降脂治疗

Effect of Glucorticoid Therapy Efficacy on Serum Lipid in Cases with Primary Nephrotic Syndrome

To understand the effect of glucorticoid therapy efficacy on serum lipid, lipoprotein and aprolipoprotein (Apo) on cases of primary nephrotic syndrome ( PNS) before and after glucorticiod treatment, the above indexes were measured with a biochemical method in 52 cases of PNS. The results showed that in the glucorticoid sensitive patients (complete remission group), the levels of TC, TG, LDL-c and ApoB 100 were significantly higher and ApoA 1/ApoB 100 was significantly lower ( P <0 01) after treatment. The above indexes were also significantly increased ( P <0 01 or P <0 05) in the partial sensitive patients (partial remission group). In the glucorticoid restant patients (non-effect group), all of the above indexes were not significantly different( P >0 05). Compared with the other two groups, the above indexes were the lowest in the complete remission group after treatment. All these suggested that the glucorticoid therapy efficacy had a significant effect on the serum lipid, lipoprotein and Apo in cases of PNS. Besides treating primary diaseases, lipids lowering therapy should be applied to the PNS patients with persistent hyperlipidemia.
