门静脉、肝静脉和下腔静脉的三维动态增强MRA:快、慢速灌注法的对比研究

摘    要:目的:比较快速和慢速注射造影剂法3DDCEMRA显示PV,HV和IVC的差别;评价3DDCEMRA在门静脉和肝脏静脉系统中的使用价值。材料和方法:本研究分两部分。首先行肝脏横断面动态扫描,测定快速法和慢速法注射造影剂PV、HV和IVC达信号峰值的时间,以确定第二部份快速法和慢速法3DDCEMRA的最佳延迟时间。然后在此基础上,我们做了40次快速法和80次慢速法3DDCEMRA成像。对两种增强方式显示血管的差异进行比较,并对各种病变的3DDCEMRA的表现做分析。结果:快速法和慢速法均能较好显示PV,显示效果相仿(p>0.05)。慢速法显示HV和IVC效果较好,优于快速法(分别为p<0.01,p<0.05);3DDCEMRA能较好显示多种疾病。结论:快速法和慢速法3DDCEMRA显示PV均较满意,但后者显示HV和IVC优于前者,对于门静脉和肝脏静脉系统多种疾病的显示和诊断具有重要价值

Abstract:Purpose:To compare the effects of rapid and slow infusion on 3D DCE MRA demonstration of PV,HV and IVC;and to evaluate the applications of 3D DCE MRA in the imaging of the portal and hepatic venous system.Materials and methods:We performed two experiments. In the first experiment,axial liver dynamic MRI with rapid and slow infusion were done to measure the mean time for reaching the peak enhancement of PV,HV,IVC,in order to determine the optimal imaging delay for 3D DCE MRA with the two infusion rates.In the second experiment,on the basis of the first,we undertook 40 3D DCE MRA with rapid infusion and 80 3D DCE MRA with slow infusion.Comparison of the two infusion rates was made in visualization of these blood vessels.3D DCE MRA manifestations of various abnormalities were presented.Results:Both rapid and slow infusion resulted in good visualization of PV.Effects of two flow rates on demonstration of PV were similar(p>0.05).For slow infusion,the demonstration of HV was good and much better than that for rapid one(p<0.01).Differences were also noted in IVC,where slow infusion was superior(p<0.05) providing good visualization of IVC.3D DCE MRA can be used in a spectrum of diseases.Conclusion:3D DCE MRA with both rapid and slow infusion can demonstrate PV satisfactorily,but the latter is superior in visualization of HV and IVC.It is considered as a valuable and alternative technique to detect and diagnose various pathologic conditions involving the portal and hepatic venous systems.
