一种晚期复发鼻咽癌（NPC）的时辰化疗方案疗效观察

目的对比观察DDP联合5－FU的时辰化疗与常规化疗在同时期化放疗应用过程中对晚期复发转移的NPC(鼻咽癌NasopharyngealCarcinoma)的疗效。方法50例局部晚期复发转移的NPC患者，随机分为时辰化疗组（CC组chrono－chemotherapy）27例，常规化疗组（RC组routine－chemotherapy）23例。均采用同时期化放疗措施。结果CC组为27例，其CR率、有效率（CR＋PR）分别为18．5％、66．7％，中位生存期12个月（3～20个月）。RC组为23例，其CR率、有效率（CR＋PR）分别为8．7％、39．1％，中位生存期10个月（2．8～17个月）。两组CR率及有效率（CR＋PR）有显著性差异(P＜0．05)，中位生存期无显著性差异（P＞0．05）。毒副反应比较结果RC组相对于CC组口腔炎、恶心、呕吐、脱发更多见（P＜0．05），骨髓毒性反应亦有显著性差异（P＜0．05）。结论DDP联合5－FU的时辰化疗对晚期复发转移的NPC治疗效果较好，毒副反应较低，因此，同时期放疗与DDP联合5－FU的时辰化疗为晚期复发转移NPC较为有效的治疗方案。

Therapeutic Effect of Chorno-Chemotherapy(DDP and 5-FU)Combined with radiotherapy for Advanced or Recurrent NPC

Objective： To compare the therapeutic effect ofchrono－chemotherapy (DDP and 5－FU) with routine－chemotherapy while radiotherapy being taken for advanced or recurrent NPC． Methods： Fifty patients with advanced or recurrent NPC were divided randomly into twe groups which included chrono－chemotherapy group (CC group) 27 cases and routine－chemotherapy (RC group) 23 cases, both were treated with chemotherapy and radiotherapy simultaneously． Results： The CR and ER of the CC group were 18．5％ and 66．7％ respectively and the median survival duration (MSD) was 12 months (3～20 months)． The CR and ER of the RC group were 8．7％ and 39．1％ respectively． The MSD was 10 months (2．8～17 months)． There was significant difference between the two groups in CR and ER (P＜0．05), but no significant difference in MSD (P＞0．05)． There were more stomatitis, nausea, vomiting and more alopecia and bone marrow suppression in RC group than those in CC group (P＜0．05)． Conclusion： The above data showed that chrono－chemotherapy (DDP and 5－FU) is more favorable in the treatment of advanced or recurrent NPC and its toxicity and side effect are weaker than that of RC． Therefore, chrono－chemotherapy appears to be one of effective regimen for advanced or recurrent NPC than routine－chemotherapy．Key Words：NPC Chrono－chemotherapy DDP 5－FU
