参附注射液、多巴酚丁胺、酚妥拉明治疗肺心病难治性心力衰竭的疗效观察

目的观察参附注射液、多巴酚丁胺、酚妥拉明治疗肺心病难治性心力衰竭的效果。方法选取肺心病难治性心力衰竭患者102例，随机分为治疗组（60例）和对照组（42例）。对照组采用常规治疗，治疗组在对照组基础上加用参附注射液、多巴酚丁胺、酚妥拉明，比较2组的疗效。结果治疗组显效37例，有效18例，无效5例，总有效率91．7％；对照组显效12例，有17效例，无效13例，总有效率69．0％。治疗组总有效率显著高于对照组，差异有统计学意义（P〈0．05）。2组治疗后呼吸、心率改善均明显优于治疗前，差异有统计学意义（P〈0．05）其治疗组明显优于对照组（P〈0．05）。结论参附注射液、多巴酚丁胺、酚妥拉明联合用药既能增加心肌收缩力，又能扩张外周血管及支气管，降低肺动脉压，还能降低血黏度，改善微循环，起到了标本兼治的作用。

Observation of curative effect of Shenfu injection combined with phentolamln and dobutamine on refractory heart failure with pulmonary heart disease

Objective To observe the curative effect of Shenfu injection combined with phentolamln and dobutamine on refractory heart failure with pulmonary heart disease. Methods 102 cases suffering from refractory heart failure with pulmonary heart disease were randomly divided into 2 groups. 42 cases in the control group were treated according to routine treatment, while 60 cases in treatment group were treated with Shenfu injection, phentolamln and dobutamine on the basis of control group. Then the curative effect was compared between two groups. Results 37 cases were improved obviously, 18 cases were improwd,5 cases were no improved and the total efficiency was 91.7% in the treatment group, versus 12 cases, 17 cases, 13cases and 69.0% in the control group. There was significant difference between two groups（ P 〈 0.05 ）. Compared with the control group, the levels of HR was significantly improved in the treatment group after treatment （ P 〈 0.05 ）. Conclusion Shenfu injection combined with phentolamln and dobutamine can decrease pulmonary artery pressure , reduce blood volume retured, enhance myocardial contractile foroe in treatment of refractory heart failure with pulmonary heart disease.
