早期联合尼莫地平治疗高血压性脑出血临床研究

目的观察尼莫地平对高血压性脑出血血肿、血肿周围低密度区影响及临床疗效。方法96例高血压性脑出血患者随机分为传统治疗组（对照组）48例和早期联合尼莫地平治疗组（治疗组）48例，分别对2组患者治疗前、治疗后3d、7d、30d行神经功能缺损评分比较，并且于治疗后30d进行临床疗效评价，同时观察2组患者血肿体积及血肿周围低密度区。结果2组治疗后3d、7d、30d神经功能缺损评分、血肿体积及血肿周围低密度区比较，治疗组优于对照组有显著性差异（P〈0．05或P〈0．01）。治疗后30d临床疗效评价，治疗组总有效率79．17％（38／48），高于对照组62．50％（30／48）有显著性差异（P〈0．05）。结论早期联合尼莫地平治疗高血压性脑出血能有效改善神经功能缺损，减少早期血肿扩大，减轻脑水肿，提高临床疗效。

Clinical Study on early administration of nimodipine in the treatment of hypertensive cerebral hemorrhage

Objective To observe the effect of nimodipine on patients with hypertensive cerebral hemorrhage. Methods 96 cases were randomly divided in two groups, including control group and therapeutic group（ nimodipine group）. The score of neurological deficits was measured before treatment, at days 3,7 and 30 after the treatment. And the clinical effect was evaluated at day 30 after the treatment. Meanwhile the volume of hematoma was observed in the two groupsl Results The score of neurological deficits and the volume of hematoma ann perilesional edema of the patients in the nimodipine group were decreased significantly when compared to that of the control patients（ P 〈0.05 or P 〈0.01 ）. The rate of effect in the theraeutic group was 79.17% （38/48）, and 62.50% （30/48）in the control group. There was a obvious defference between the effect of the two groups （P 〈 0.05）. Conclusion Nimodipine can reduce the volume of hematoma and perilesional edema, and improve neurological deficits in the early phase of hypertensive cerebral hemorrhage.
