心踝血管指数与高同型半胱氨酸血症的相关性研究

摘    要:目的探讨血管性疾病患者心踝血管指数(CAVI)与高同型半胱氨酸血症(HHcv)之间的相关性。方法选择高血压、冠心病患者63例,根据血浆同型半胱氨酸(Hcy)水平分为对照组(35例)及HHcv组(28例)。应用动脉僵硬度自动检测装置测定CAVI作为反映动脉僵硬度的指标。结果HHcy组CAVI、颈一股脉搏波传导速度(CFP.WV)显著高于对照组(9.59±2.47VS.7.77+1.86,P=0.002;13.54+3.28VS.10.63_+2.65,P〈0.001)。63例患者中血浆Hey与年龄、CFPWV、CAVl分别呈正相关(r分别为0.357,0.331,0.417;P均〈0.05)。校正年龄、性别因素后,Hcv与CFPWV、CAVI仍呈正相关(r=0.260及0-316.P均〈0.05)。多元线性回归分析显示Hcv是CAVI的独立影响因素(β=0.447,P=0.013)。结论HHcv患者CAVI水平显著升高,提示Hcv参与了动脉僵硬度的发生进展过程。

Possible link between cardio-ankle vascular index and homocysteine in patients with vascular related diseases

Abstract:Objective To investigate the possible link between cardio-ankle vascular index and homocysteine (Hcy) level in vascular related diseases. Methods Sixty-three patients (M/F: 30/33, age: 64.6_+13.7ys) with hypertension or coronary artery disease (CAD) from Peking University Shougang Hospital Vascular Medicine were enrolled into our study. They were divided into two groups: the control group (n=35), and the hyperhomocysteinemia (HHcy) group (n=28). The VS-1000 was used to measure CAVI. Carotid-femoral pulse wave velocity (CFPWV) was detected by the Complior apparatus. Results The value of CAVI, CFPWV were higher in the HHcy group than in the control group (9.59_+2.47 vs. 7.77_+1.86, P = 0.002; 13.54_+3.28 vs. 10.63_+2.65, P 〈 0,001). However, the prevalence of hypertension or CAD was not significantly different between these two groups. Hcy was positively correlated with age, CFPWV, CAVI in the entire study group (r = 0.357,r = 0.331 ,r = 0.417, all P 〈 0.05) even after adjusting for age and gender. Multivariate analysis showed that Hcy was an independent associating factor of CAVI in all subjects (β = 0.447, P = 0,013). Conclusion CAVI is significantly higher in hyperhomocysteinemia patients indicating that Hcy may play a role in the development of arterial stiffness.
