微创经皮肾镜治疗肾结石75例疗效观察

目的：探讨应用B超引导微创经皮肾镜碎石术（MPCNL）治疗上尿路结石的方法、疗效和安全性。方法：采用B超定位引导下对75例上尿路结石采用经皮肾镜碎石，并对手术操作方法、并发症进行分析。结果：75例患者均为单通道取石，69例均一期取尽结石，6例在二期取尽结石，平均手术时间（95±25）min，出血量（105±54）ml，肾造瘘管留置时间平均为（7.9±2.4）d，住院时间（12.7±3.1）d。1例胸腔积液，3例腹腔积液，进行碎石清除术。结论：B超引导下微创经皮肾镜气压弹道碎石术治疗上尿路结石操作方法安全可靠、损伤小、出血少、穿刺成功率高、并发症少。

Evaluation of the efficacy of minimal invasive percutaneous nephrolithotomy in 75 cases

Objective：To investigate the feasibility and effects of minimally invasive percutaneous nephrolithotomy under B ultrasound guidance for nephrolithiasis.Methods：We retrospectively analyzed 75 patients treated with minimally invasive percutaneous nephrolithotomy under B ultrasound guidance for nephrolithiasis .Results：Single tract was in all cases. In 75 cases.92%stone-free rate were achieved using one-stage minimally invasive percutaneous nephrolithotomy.The mean operative time was 95 ±25 minutes,the nephrostomic tubing mean duration was 7.9 ± 2.4days. The hospitalization days were 12.7 ±3.1 days, the estimated blood loss volume 105 ±54ml （mean volume 105ml）.The complication were thorax hydrocele in one patient and celiac hydrocele in two cases.Conclusion：Minimally invasive PCNL is an effective and safe option for the treatment renal calculi.it may offer advantages with respect to less intraoperative and postoperative complications,less invasion,easy recovery and short hospitalization stay.
