低场MRI在膝关节隐匿性骨折中的实验研究

摘    要:目的:探讨低场MRI对膝关节隐匿性骨折的诊断价值。方法:实验羊膝关节3对,使每对中一只人工用钝器横向适度砸伤,且于受伤后6~12 h将羊宰杀后取出双膝关节,行DR摄片及低场MRI常规序列检查,然后送病理切片行常规HE染色检测观察。结果:3例实验羊膝关节隐匿性骨折患者T1WI呈低信号,或内见高信号,T2*OWI呈低或高信号,T2WI呈等或高信号,在WFT2*水像或STIR序列上病变较正常的骨组织信号更高,呈高信号。病理切片HE显示羊膝关节镜下可见明显骨小梁断裂,周围软组织高度水肿、渗出及出血,骨板结构正常。结论:低场MRI能清楚显示膝关节隐匿性骨折的病变特点及严重程度,可作为膝关节隐匿性骨折的重要检查方法。

Abstract:Objective: To explore the value of low-field MR1 in diagnosis of occult fracture of knee joint. Methods: Three sheep knee joints suffering from trauma for 6-12h were used for DR radiography,routine sequence check of low-field MRI and HE staining. Results:Three cases with occult knee joint fractures present low or internal high signal on T1 WI,low or high signal on T2 ^*OWI, equal or high signal on T2 WI and higher signal on WFT2^ * water or STIR sequence than normal bone tissue. Obvious fracture of trabeeular bone, soft tissue edema and effusion as well as bleeding were observed in the sheep knee joint under microscope by HE staining, but the hone plate structure was normal. Conclusion: Low-field MRI can clearly show the pathological characteristics and degree of occult knee joint fractures, therefore,it can be used as an important method for the inspection of occult knee joint fractures.
