突出的腰椎间盘退变程度病理学初步研究

目的 探讨突出的腰椎间盘退变程度的超微病理学持征及临床意义。方法 应用透射电镜技术观察20例突出椎间盘的纤维环及髓核组织，并结合临床资料进行分析。结果 （1）椎间盘组织退变程度可根据细胞及胶原纤维超策结构变化来判断。（2）细胞与胶原纤维退变程度一致；（3）破裂型与非破裂型椎间盘组织在退变程度上无明显相关；（4）退变程度与年龄及病程长短明显相关，而与症状的严重程度无相关。结论 电镜下腰椎间盘超微结构改变是判断椎间盘退行性变的可靠指标。退变程度与年龄及病程长短明显相关而与临床表现无关。

A Preliminary Pathological Study on the Degradation Degrees of Herniated Lumbar Intervertebral Disc

Objective To investigate the pathological characteristics and clinical significance of herniated lumber intervertebral disc. Methods Combining with clinical data, 20 cases of lumber intervertebral fibroannulus and nucleus pulposuses were observed under an electron microscope. Results (1)The degrees of degradation can be identified by ultrastructural changes of cell and collagen fiber. (2)The degree of cell degradation was parallel to that of ultrastructural changes in collagen fiber. (3)There was no significant relationship between the broken intervertebral discs or without broken. (4)The degree of degradation was closely associated with age and course, but not severity of clinical manifestations. Conclusion The ultrastrustural change of intervertebral disc is a reliable indicator to identify the degree of its degradation, which is closely associated with age and course, but not severity of clinical manifestations.
