前路减压植骨内固定治疗胸腰椎爆裂骨折

摘    要:目的探讨前路减压、植骨融合及钛钢板内固定治疗胸腰椎爆裂型骨折的临床效果和应用价值。方法2004-01/2006-12,采用前路减压、植骨及钛钢板固定术治疗胸腰椎爆裂型骨折42例,并截瘫32例,椎管骨性占位、硬膜受压,但无神经症状10例。结果随防0.5~2.1年,平均1.3年。伤椎高度恢复90%30例,恢复80%12例,无钢板螺钉断裂和松动现象。截瘫者术后脊髓神经功能按Frankel分级,除A级2例未能恢复外,其余恢复1~2级。结论胸腰椎前路减压植骨钛钢板固定具有减压彻底、固定牢靠、并发症少、力学性能好等优点,是治疗胸腰椎爆裂型骨折较理想的方法。

Abstract:Objective To study the clinical outcome of thoracolumbar burst fractures treated by anterior route decompression bone grafting and internal fixation by plate system. Method Jan, 2004 - Dec. 2006, forty-two cases of thoracolumbar burst fractures were studied. Thirty-two of them are suffered with Parap legia. All cases were operated upon by anterior decompression, reduction, autogenous iliac grafting and fixation by plate fixation system. Result All cases were followed up for 0.5 - 2. 1 years with an average of 1.3 years. No complications occurred, such as breaking of the plate and screw. 30 cases of the height of fracture vertebra were recovered 90%, and 12 cases were recovered 80%. All the patients were recovered 1 to 2 Frankel grades, except 2 cases whose spinal nervus function were grade A. Conclusion Plate system has the advantages of simple manipulation, intrinsic stability, less complications and good biocompatibility, and had good internal fixation in the treatment of thoracolumbar burst fractures.
