脑出血患者脑脊液中弓形虫的检测及临床意义

摘    要:目的探讨弓形虫感染与自发性脑出血的关系。方法选取155例自发性脑出血患者设为研究组,149例急性脑外伤患者设为对照组,比较两组间脑脊液的弓形虫感染率。应用酶联免疫吸附实验(ELISA)测定脑脊液中弓形虫特异性抗体IgG和IgM以及循环抗原CAg的阳性率。结果研究组和对照组的脑脊液中,弓形虫抗体IgG的阳性率分别为15.48%和5.47%;弓形虫抗体IgM的阳性率分别为25.81%和6.04%;弓形虫CAg循环抗原的阳性率分别为0和0。研究组脑脊液的IgG、IgM阳性率与对照组比较,差异有统计学意义(P<0.05)。研究组脑脊液的CAg循环抗原阳性率与对照组比较,差异无统计学意义(P>0.05);结论弓形虫阳性感染者较阴性感染者更易发生自发脑出血,筛查脑脊液中弓形虫IgG、IgM抗体,可能有助于临床自发性脑出血的评估。

Detection and Clinical Significance of Toxoplasma Gondii in Cerebrospinal Fluid of Cerebral Hemorrhage Patients

Abstract:Objective To explore the association between toxoplasma gondii infection and spontaneous cerebral hemorrhage. Methods 155 case spontaneous cerebral hemorrhage patients were selected as research group and 149 case acute brain trauma patients were selected as control group. Specific antibody IgG and IgM of toxoplasma gondii and CAg antigen of two group patients were determined by ELISA. Results Ratio of positive toxoplasma gondii IgG antibody of two group patients'cerebmspinal fluid is respectively 15.48% and 5.47% ;ratio of positive toxoplasma gondii IgM antibody of two group patients' cerebrospinal fluid is respectively 25.81% and 6.04%. positive sample of toxoplasma gondii CAg circulating antigen in two group patients' cerebmspinal fluid is respectively 0 and 0. Ratio of IgG, IgM antibody was significant higher in research group than in control ( P 〈 0.05), Ratio of CAg circulating antigen revealed no statistically significances between research group and control ( P 〉 0.05). Conclusion Spontaneous cerebral hemorrhage were more declined in group with toxoplasma gondii infection than in group without toxoplasma gondii infection. It maybe hopeful to screen CSF IgG, IgM of toxoplasma for evaluation of spontaneous cerebral hemorrhage.
