乳腺癌循环肿瘤细胞与原发肿瘤临床病理特征关系的初步研究

摘    要:目的 探讨乳腺癌循环肿瘤细胞(circulating tumor cell,CTC)与原发肿瘤临床病理特征的关系.方法 128例乳腺癌及30例乳腺纤维腺瘤患者首次住院时抽取静脉血5 ml,用密度梯度离心法富集分离单个核细胞,以免疫荧光法检测所获单个核细胞中细胞角蛋白(cytokeratin,CK)8/18阳性的CTC,分析CTC与患者临床病理特征的关系.结果 在128例乳腺癌患者中CTC阳性率为40.6%(52/128),30例乳腺纤维腺瘤患者均未检出CTC.不同淋巴结病理分期(pN0、pN1、pN2和pN3期)患者CTC阳性率分别为15.4%(6/39)、27.O%(10/37)、64.7%(11/17)和66.7%(12/18),各组CTC阳性率差异有统计学意义(P=0.000 6);转移性(M1期)乳腺癌患者CTC阳性率为80%(16/20),较非转移性(M0期)乳腺癌患者CTC总阳性率33.3%(36/108)显著升高(P=0.000 9);小同TNM分期(Ⅰ、Ⅱ、Ⅲ和Ⅳ期)患者CTC阳性率分别为12.5%(3/24)、32.0%(16/50)、50.0%(17/34)和80.0%(16/20),各组CTC阳性率差异有统计学意义(P=0.000 3).结论 部分乳腺癌患者血液中存在CTC.CTC与淋巴结转移、远处转移及TNM分期密切相关.检测CTC有利于进行更精确的病情评估.
