BiPAP呼吸机模式对心脏术后急性心力衰竭的治疗作用

目的探讨无创性双水平气道正压(BiPAP)呼吸机面罩式双水平气道正压通气治疗心脏术后急性心力衰竭的疗效。方法对26例心脏术后急性心力衰竭患者常规抗心力衰竭治疗同时加用BiPAP 呼吸机面罩式压力支持通气；进行呼吸频率、心率、动脉血气分析及临床症状体征等监测,并进行相关分析。结果 26例患者临床症状明显改善,PaO_2、SaO_2等与治疗前比较显著增高(P＜0.01),双肺湿罗音明显减少,心率、呼吸频率恢复正常。结论在常规抗心力衰竭治疗的基础上,短期应用BiPAP正压呼吸机通气治疗,能迅速改善心功能,不良反应少,是抢救急性心力衰竭的一种安全有效的方法。

Efficiency of BiPAP respirator in managing acute cardiac failure after cardiac surgery

Objective To investigate the efficiency of mask BiPAP respirator positive airway pressure support in managing acute cardiac failure following cardiac surgery.Methods The 26 patients with acute cardiac failure after cardiac surgery were treated with BiPAP respirator in addition to conventional treatment. The changes of clinical symptoms, arterial blood gas(pH,PaO_2,PaCO_2)and BP were measured to evaluate the efficiency of BiPAP respirator.Results After using BiPAP respirator, the clinical symptoms had been improved significantly; all the clinical parameters showed a statistical difference compared with those before BiPAP (P<0.01).Conclusion BiPAP respirator can improve symptoms and hypoxemia in patients with the servere acute heart failure after cardiac surgery. It is a safe and effective method in treating the acute heart failure after cardiac surgery.
