高压氧治疗激素诱导兔股骨头缺血性坏死的病理观察

摘    要:目的探讨高压氧(hyperbaricoxygen,HBO)治疗激素诱导股骨头缺血性坏死(steroidinducedavascularnecrosisofthefemoralhead,SANFH)的病理改变。方法采用大剂量醋酸泼尼松龙诱发成年兔SANFH,然后用HBO治疗;应用光镜和电镜观察组织病理学及超微结构变化。结果多数骨小梁周围可见骨母细胞增生、偶见破骨细胞增生,髓腔内脂肪细胞逐渐减少,造血组织增生;电镜发现骨小梁内早期即出现幼稚骨细胞和胶原原纤维合成。结论SANFH经HBO治疗后增强破骨细胞和成骨细胞活性以及胶原纤维生成,有助于降低骨内压、改善微循环、促进骨组织修复。

Abstract:Purpose To investigate the pathogenesis of steroid-induced avascular necrosis of femoral head(SANFH) treated with hyperbaric oxygen(HBO) therapy. Methods Pathological changes of femoral head were achieved by administration of prednisolone followed by treatment with HBO therapy. Pathologic and ultrastructural changes were observed with light microscope and electron microscope. Results Osteoclast and osteoblast around trabecula of bones showed proliferation, while fatty tissues in bone marrow decreased and hematopoietic tissue well-regenerated after the hyperbaric oxygen-treated. Also juvenal osteocyte and collagen fibril were observed in early stage by electron microscope. Conclusions The results suggested that SANFH treated with HBO therapy may enhance the osteoclastic, osteoblastic activities and collagen fibers formation. It is helpful to depress intraosseous pressure, improve microcirculation and tissue repair.
