原发性醛固酮增多症诊治分析

目的：提高对原发性醛固酮增多症（PA） 的诊断和治疗水平.方法：对本院1999年8月-2009年5月经临床确诊的83例PA患者的血压水平、血管并发症发生率、生化检查、与治疗效果进行回顾性分析.结果：83例PA患者均为持续性高血压,15.7%（13/83）为1级高血压,48.2%（40/83）的患者为2级高血压,36.1%（30/83）的患者为3级高血压.心脏彩超提示左心室肥厚患者占38.5% （32/83）,脑血管并发症占7.2% （6/83）,出现肾损害者占14.5%（12/83）.患者手术后血压正常率为62.6%.结论：原发性醛固酮增多症患者大多数为中重度高血压,血管并发症常见,外科治疗对降血压的效果需结合患者具体情况综合分析.

Diagnosis and treatment of primary aldosteronism

Objective：To improve the diagnosis and treatment of primary aldosteronism （PA）. Methods：Data of 83 PA patients in our hospital from August of 1999 to May of 2009 ,including blood pressure,vessel complication incidence, biochemistry examination results and treatment effectiveness were retrospectively analyzed. Results：All of the 83 PA patients had continuous hypertension, 15.7% （13/83） of whom were with grade Ⅰ hypertension,48.2% （40/83） were with grade Ⅱ hypertension and 36.1% （30/83） were with grade Ⅲ hypertension. According to color cardiac ultrasound, the incidences of left ventricular hypertrophy, cerebrovascular complication and renal damage were 38.5% （32/83） ,7.2% （6/83） and 14.5% （ 12/83 ） respectively. The rate of normotension after operation was 62.6%. Conclusion： Most PA patients had moderate to severe hypertension, and vascular complications were rather common. Aggregate analysis to patient＇ s individual condition is required for evaluating the anti-hypertension effect of surgical therapy.
