心力衰竭患者血清CA125和血浆BNP水平变化的探讨

目的：研究心力衰竭（HF）患者血清CA125浓度变化情况,探讨血清CA125在HF患者疾病监测、治疗中的临床价值。方法：161例临床确诊为HF的患者作为研究对象,根据并按照纽约心脏学会（NYHA）分为Ⅰ级～Ⅳ级,用化学发光免疫法检测血清CA125和血浆BNP的浓度,并选择性别、年龄相匹配的30例排除特征的普通病人作为对照。结果：在HF组中,Ⅳ级、Ⅲ级的血清CA125浓度较对照组显著升高（P〈0.05）,血清CA125浓度与左室射血分数呈负相关（P〈0.05）,与血浆BNP浓度呈正相关（P〈0.05）,含有胸腔或是心包积液患者血清CA125和血浆BNP浓度明显高于不含积液的患者（P〈0.05）。结论：HF患者血清CA125浓度与NYHA分级有明显相关性,可作为判断HF严重程度、预测疾病进展的实验室指标。

Study on the Changes of Serum CA125 and Plasma BNP Levels in Patients with Heart Failure

Objective To investigate serum carbohydrate antigen 125(CA125) and plasma BNP levels in patients with heart failure(HF)and to discuss the clinical value in the monitoring and treatment of HF patients.Methods The patients were divided into four classes according to their New York Heart Association(NYHA)functional class.Serum CA125 and plasma B-type natriuretic peptide(BNP) of 161 patients admitted for HF and 30 persons as common patients without characteristics control was measured by chemiluminescence immunoassay.Results Serum level of CA125 was significantly higher in class Ⅳ,Ⅲ than that in control group.There was negative correlation between serum level of CA125 and left ventriculer ejection fraction(LVEF),but positive correlation between serum CA125 and plasma BNP.Serum CA125 and plasma BNP levels of patients with thoracic cavity or pericardium effusion were higher than the patients without effusion.Conclusion There is a positive correlation between serum level of CA125 and NYHA functional class,and serum level of CA125 can be used as a laboratory maker to judge the disease severity and predict the progress of HF.
