嗜铬细胞瘤患者血浆和24h尿CAs水平变化及临床应用分析

摘    要:目的:探讨嗜铬细胞瘤患者血浆、24h尿儿茶酚胺(CAs)水平变化与诊断、治疗及预后相互关系。方法:用酶联免疫分析(血浆)和高效液相色谱分析(24h尿)的方法,对97嗜铬细胞瘤患者、30例正常对照者,血浆及24h尿液中儿茶酚胺含量进行检测分析。结果:正常对照组与嗜铬细胞瘤患者组比较,血浆和24h尿儿茶酚胺(E、NE、DA)都明显高于正常对照组,有显著性差异(P<0.01);嗜铬细胞瘤患者血浆与24h尿儿茶酚胺(E、NE、DA)水平变化呈明显的正相关(r①=0.892,P<0.001;r②=0.781 P<0.001;r③=0.912,P<0.001);嗜铬细胞瘤患者手术治疗后跟踪随访血、尿CAs水平与正常对照组无明显差异。结论:嗜铬细胞瘤患者血浆和24h尿CAs水平变化与诊断、治疗及预后密切相关,24h尿CAs检测优于血浆,检测结果对临床早期诊断、治疗及预后判断具有重要意义。

Changes and Clinical Application of Serum and 24-hour Urine Catecholamine Levels in Patients with Pheochromocytoma

Abstract:Objective To explore the relationship between the changes of plasma CAs,24-hour urine CAs levels and the diagosis,treatment,prognosis of pheochramocytoma patients.Method Using ELISA and high performance liquid chromatography(24h urine) to analyze plasma and 24-hour urine catecholamine levels in 97 pheochromocytoma patients and 30 normal controls.Results Plasma and 24h urine catecholamine(E,NE,DA) were markedly higher in the group of pheochromocytoma patients than those in the normal control group(P<0.01);level changes of catecholamines(E,NE,DA) in plasma and 24h urinary of pheochromocytoma patients showed a positive correlation(r=0.892,P<0.001;r=0.781,P<0.001;r=0.912,P<0.001).There were no significant differences in plasma and 24h-urine CAs level between pheochromocytoma patients after surgical treatment and normal group.Conclusion The changes of plasma and 24-hour urine(CAs) levels are closely related to diagnosis,treatment and prognosis in pheochromocytoma patients.Determination of 24h-urine CAs is superior to plasma CAs,the test results are of great significance for the early diagnosis,treatment and prognosis.
