围手术期输血对直肠癌根治术后的影响

目的探讨围手术期输血对直肠癌术后复发及预后的影响。方法回顾性分析本院1994年6月至2003年6月收治的156例直肠癌DuekesC期根治手术病例输血情况和随访结果，比较输血组与未输血组术后复发情况及预后情况。结果输血组术后复发率较未输血组高（P=0.034），输血量〉600ml组比输血量〈600ml组复发率高，差异有统计学意义（P=0.037）。输血组与未输血组生存率之间比较差异无统计学意义（P=0.458）。结论输血可增加直肠癌患者术后复发率，缩短术后无瘤生存期，临床用血应仔细权衡。

The influence of preoperative blood transfusion on the recurrence and prognosis of patients with rectal carcinoma undergoing radical resection

Objective To investigate the influence of preoperative blood transfusion on the recurrence and prognosis of patients with rectal carcinoma undergoing radical resection. Methods Clinical and follow-up date of 156 patients with rectal cancer （Duckes C） , received rectal resection from Jun. 1994 to Jun. 2003 in our hospital were analysed respectively, and the recurrence rate and prognosis of the two groups were compared. Results The recurrence rate of the group of blood transfusion was higher than the group of not blood transfusion （P=0.034）, and the same outcome between the two groups of blood transfusion, the recurrence rate of those who had blood transfusion 〉 600 ml was higher than those who had blood transfusion 〈 600 ml （P=0.037）, there was a significant between the two groups above. No significant difference was detected in terms of the 5-year survival rate between the group of blood transfusion and the group of not blood transfusion （P=0.458）. Conclusion Blood transfusion is closely related with the recurrence rate of patients with rectal carcinoma after radical resection, we should consider carefully before blood transfusion.
