二维及彩色多普勒超声对下肢深静脉血栓的诊断价值

摘    要:目的 :探讨二维及彩色多普勒血流成像 ( 2D -CDFI)在下肢深静脉血栓 (DVT)形成中的应用价值。方法 :通过二维和彩色多普勒对 15 8例急、慢性下肢深静脉血栓病人的诊断进行分析。结果 :急性血栓 12 7例 ,位于股静脉68例 ( 5 3 .5 % ) ,静脉 3 9例 ( 3 0 .7% ) ,2 0例 ( 15 .7% )为其他部位的血管。超声二维表现为静脉管腔增宽 ,内见低回声的血栓 ;彩色多普勒检测管腔内无血流或血流充盈缺损。脉冲多普勒 (PW )示无血流信号或低速血流频谱 ,且波形不随呼吸而变化。慢性血栓 3 1例 ,二维图像表现为血管粗细不均 ,管腔内为稍强回声 ,2 7例彩色多普勒见“轨道征”血流 ,2 6例有侧支循环形成。结论 :2D -CDFI对指导下肢DVT临床用药并观察疗效有重要价值

Abstract:Objective:To evaluate application of two-dimentional echo (2D) and color doppler flow imaging (CDFI) in diagnosis of deep venous thrombosis of lower extremites.Methods:2D and CDFI was applied to diagnose 158 patients with deep lower venous thrombosis.Results:There were 127 cases with acute venous thrombosis, of whom 68 cases were afflicted with femoral venous thrombosis (53.5%), 39 cases with popliteal venous thrombosis(30.7%),20 cases with others venous thrombosis(15.7%). 2D shows enlarged vein, hypoecho thrombosis and all or part shrunk. venous diameter. There were no or little flow in vein with CDFI and PW. Somewhat little hyperecho and unequal venous diameter in 31 cases with chronic venous thrombosis, of whom the color blood flow of "railway sign" was shown in 27 cases and the compensatory circulation was formed in 26 cases.Conclusions:2D and CDFI, as a highly sensitive, specific and non-invasive procedure used at the early stage to diagnose deep lower vena thrombosis ,has great value to clinical practice and evaluation of therapeutic efficacy.
