乌司他丁对急诊腹部手术患者机体应激反应的影响

目的　探讨乌司他丁对急诊腹部手术患者应激反应的影响。方法　 4 8例急诊腹部手术患者随机分为观察组和对照组 ,每组 2 4例。观察组术后每日经静脉滴注乌司他丁 2 0万U ,共 3天。对患者术前、术后应激反应的部分指标 :白细胞计数、C 反应蛋白、血糖、前白蛋白和皮质醇浓度进行测定。结果　术前各项指标无差异 ,术后血清白细胞计数、C 反应蛋白、血糖和皮质醇浓度观察组均明显低于对照组 (P <0 .0 5 ) ,前白蛋白浓度观察组明显高于对照组 (P <0 .0 5 )。结论　术后应用乌司他丁能减轻机体的应激反应 ,稳定内环境 ,有利于急诊腹部手术患者术后的康复

Effect of ulinastation on stress in emergency abdominal surgery patients

Objective To observe the effect of ulinastation on stress in emergency abdominal surgical patients.Methods 48 patients undergoing emergency abdominal surgery were randomly divided into two groups(n=24): observation group and control group.Ulinastation(200000U iv gtt,qd) was given to the observation group for 3 consecutive days after operation.The parameters including leukocyte count,C reaction protein,glucose,prealbumin and hydrocortisone were measured and analyzed.Results Postoperative leukocyte count,serum concentration of C reaction protein,glucose and hydrocortisone in observation group were significantly lower than those in control group(P<0.05) while the serum level of prealbumin in observation group was significantly higher than that in control group(P<0.05).Conclusion Postoperative ulinastation may downregulate the stress and stabilize inner circumstance,which will be helpful to the patients recovery after emergency abdominal surgery.
