中心静脉导管集束干预策略的实施依从性及有效性研究

目的：探讨中心静脉导管集束干预策略临床实施的依从性及对中心静脉导管相关性血流感染（CRBSI）的预防作用。方法：采用干预前后对照研究的方法,将2011年4月-2012年3月采用传统的中心静脉导管管理方法的患者作为对照组,共110例。将2012年4月-2013年3月采用中心静脉导管集束化干预策略管理方法的患者作为观察组,共131例。观察指标：中心静脉导管集束化干预策略执行依从性、导管使用率和每千个导管日CRBSI感染率。结果：实施中心静脉导管集束化干预策略后,导管使用率和每千个导管日CRBSI感染率均下降,中心静脉导管集束化干预策略执行依从性上升。结论：实施中心静脉导管集束化干预策略（CLB）能降低CRBSI的发生率,同时,其临床实施依从性将直接影响患者的导管留置时间和CRBSI的发生率。

Clinical Study on the compliance and the validity of central line bundle

Objective： To study the clinical compliance of central line bundle implementation and its preventive effect on Catheter-Related Blood Stream Infection. Methods： A before and after design was used in this single center study,110 cases from April,2011 to March,2012 were put to use Management methods of central venous catheter with traditional,131cases between April,2012 to March,2013 were put to use central line bundle. Observing indexes： the compliance of central line bundle implementation,the usage rate of central line,the incidence rate of CRBSI on every thousand of catheter indwelling day. Results： The usage rate of central line and the infection rate of the average catheter indwelling day were down after central bundle implementation. The performing compliance of central line bundle implementation was raised after central bundle implementation. Conclusion： The incidence of CRBSI was down after bundle implementation,and the compliance of central line bundle directly impacts the usage rate of central lines and the incidence of CRBSI.
