阴道彩色多普勒超声对绝经后子宫内膜病变的诊断价值

目的探讨阴道彩色多普勒超声对绝经后子宫内膜病变的诊断价值。方法选择2010年1月至2013年4月深圳市龙岗区第二人民医院收治的120例绝经后子宫内膜病变患者作为研究对象,所有患者均进行阴道彩色多普勒超声检查,测量所有患者子宫内膜厚度,判断病变区域血流情况。结果 120例子宫内膜病变中,良性病变98例,恶性病变22例,良性病变患者子宫内膜厚度小于恶性病变患者(7.2±3.8)mm vs(10.8±4.3)mm,P<0.01];36例萎缩性子宫内膜未见血流,其余62例良性病变患者子宫内膜血流阻力指数大于恶性病变患者(0.61±0.07)vs(0.34±0.05),P<0.01],舒张期末流速低于恶性病变患者(8.6±3.4)cm/s vs(14.7±5.6)cm/s,P<0.01],收缩期峰值流速与恶性病变患者差异无统计学意义(25.8±5.2)cm/s vs(27.9±6.6)cm/s,P>0.05]。结论阴道彩色多普勒超声检查分辨率高、操作简单、无创伤,可作为绝经后子宫内膜病变首选的检查方法。

Study on the Diagnosic Value of Transvaginal Color Doppler Ultrasonography in the Treatment for Endometrial Lesions in Postmenopausal Women

Objective To investigate the diagnostic value of transvaginal color Doppler ultrasonography in the treatment for endometrial lesions in postmenopausal women. Methods A total of 120 postmenopausal patients with endometrial lesions admitted in The Second People ＇s Hospital of Longgang District from Jan.2010 to Apr. 2013 were chosen as the study objects and all the patients underwent transvaginal color Doppler ultrasound examination,the endometrial thickness of all patients were measured to identify the blood flow in pathological field. Results Among all the cases,98 cases were benign lesions,22 cases were malignant lesions,endometrial thickness of benign lesions in patients was less than that of the patients with malignant lesions（ 7. 2 ± 3. 8） mm vs（ 10. 8 ± 4. 3） mm,P〈0. 01]; no blood flow was found in 36 cases with atrophic uterine endometrium,in the other 62 cases with benign lesions,endometrial blood flow resistance index was greater than that of patients with malignant lesions（ 0. 61 ± 0. 07） vs（ 0. 34 ± 0. 05）,P〈0. 01],and EDV was lower than that of the patients with malignant lesions（ 8. 6 ± 3. 4） cm / s vs（ 14. 7 ± 5. 6） cm / s,P〈0. 01],the difference of PSV between the patients with benign lesions and malignant lesions had no statistical significance（ 25. 8 ± 5. 2） cm / s vs（ 27. 9 ± 6. 6） cm / s,P〉0. 05]. Conclusion Transvaginal color Doppler ultrasonography has the characteristics of high resolution,simple operation,and no trauma,therefore,it can be used as the preferred inspection method for postmenopausal endometrial lesions.
