股骨近端防旋髓内钉内固定治疗高龄患者股骨粗隆间骨折的临床效果分析

目的分析股骨近端防旋髓内钉(PFNA)内固定治疗高龄股骨粗隆间骨折的临床疗效。方法将2013年1月至2014年1月吉林油田总医院收治的60例高龄股骨粗隆间骨折患者按随机数字表法分为两组:对照组(30例)采用动力髋螺钉(DHS)法治疗,PFNA组(30例)采用PNFA法治疗。记录两组患者的术中及术后情况,并进行比较分析。结果 PFNA组患者的手术时间为(55.2±9.8)min、术中出血量为(98.1±23.2)m L,均低于对照组患者手术时间(111.2±10.2)min、术中出血量(221.3±28.7)m L(P<0.01)。Harris标准评分,PFNA组优良率为86.7%,对照组患者优良率为66.7%,但2组差异无统计学意义。结论 PFNA内固定法具有创伤小、固定牢靠、并发症少的优点,更适用于高龄患者股骨粗隆间骨折的治疗。

The Analysis of the Clinical Effect of Proximal Femoral Nail Antirotation Internal Fixation Treatment for Elderly Patients with Intertrochanteric Fracture

Objective To analyze the clinical effect of proximal femoral nail antirotation internal fixation treatment for elderly patients with intertrochanteric fracture. Methods A total of 60 cases of intertrochanteric fracture in elderly patients admitted to the Qilfields General Hospital of Jilin Province from Jan. 2013 to Jan. 2014 were divided into two groups by random number table method,30 case of the control group were given dynamic hip screw treatment and 30 case of PFNA group were given PNFA treatment. The intraoperarive and postoperative situation of the two groups was comparatively analyzed. Results The operation time （55.2±9.8 ） rain , surgical blood loss （ 98.1±23.2 ） mL, operation time and surgical blood loss of the PFNA group were lower than the control group （ 111.2±10.2 ） rain and （221.3±28.7 ） mL（ P 〈 0.05 ）. Harris standard score of ＂excellent and good＂ of the control group patients was 66.7% ,of the PFNA grour was 86.7% and the difference was not statistically significant. Conclusion PFNA internal fixation methoc has the advantages of small trauma, solid fixation and less complications, which is more suitable for the treatment of senile patients with intertrochanteric fracture.
