子宫腺肌病的治疗概况

子宫腺肌病(AM)的发病机制尚不明确,至今无理想的根治方法。目前常用的药物治疗有雄激素类衍生物、促性腺激素释放激素激动剂、孕激素受体水平拮抗剂、孕激素、避孕药、芳香化酶抑制剂、中药等;手术治疗主要有保守手术及根治手术;介入治疗主要有子宫动脉栓塞术、超声引导下经皮腹腔穿刺微波凝固治疗、聚焦超声及射频消融等。近年来药物治疗、微创治疗等方面取得的可喜进展为AM的治疗提供了更多的选择,但目前子宫切除术仍是根治性治疗AM的唯一选择。

Study on the Treatment for Adenomyosis

Adenomyosis（ AM） is a common gynecopathy,but its pathogenesis is not clear,so far there is no ideal treatment method. The main treatment methods include drug therapy,surgical treatment and intervention therapy. Commonly used drug treatment includes androgen derivatives,gonadotropin-releasing hormone agonist,progesterone receptor antagonist,progesterone,contraceptives,aromatase inhibitors（ AIs）,traditional Chinese medicine,etc. Surgical treatment mainly includes conservative surgery and radical surgery. Interventional therapy mainly includes uterine artery embolization,the abdominal ultrasound guided percutaneous puncture microwave coagulation therapy,focused ultrasound and radiofrequency ablation,etc. Each treatment has its advantages and disadvantages,in recent years,drug therapy,minimally invasive treatment have made great research progress so as to provide more choices of treatment for AM,but hysterectomy still remains the only option of radical treatment for AM.
