阴道成形术66例分析

目的 比较五种阴道成形术的优缺点，寻找损伤小．疗效好，操作简单的手术方式。方法 1988年1月至2004年12月，对66例先天性无阴道患者，分别采用腹膜、羊膜、胎儿皮肤、阴股沟皮瓣、大腿内侧皮瓣行阴道成形术，对手术时间、住院时间、供皮区切口愈合和阴道湿润度及性生活满意度进行对比观察。结果 腹膜、胎儿皮肤及羊膜成形术与阴股沟皮瓣及大腿皮瓣成形术相比，人工阴道湿润．性生活满意度较高．且胎儿皮肤及羊膜成形术手术时间短、无损伤。结论 腹膜、羊膜及胎儿皮肤阴道成形术是五种阴道成形术中较理想的术式。

Comparison of types of vaginoplasty

Objective To search for a satisfactory surgical apptoach in vaginoplasty with minimum injury and maximum effectiveness. Methods From January 1988 to December 2004,66 cases of congenital absence of vagina were treated by five types of vaginoplasty, using peritonuem, fetal skin graft, ammiotic membrane, vulvar-inguinal skin flap,leg skin flap respectively. The duratian of operatian and the hospitalization, the wound healing of donor site, as well as the mostsensation of the artificial vagina and the sezual life quality were compared among five types of vaginoplasty. Results Compared with those by valvar-inguinal skin flao and leg skin flao, the vaginopasty by peritoneum,fetal skin graft and ammiotic membrane had the best moistsensation and the most satisfactory sexual life quality;The vaginopasty by fetal skin graft and ammiotic membrane had the shortest surgical durating, had not injury;The duration of hospitalization with five types almost the same. Conclusion The vaginoplasty by peritoneum, fetal skin graft and ammiotic membrane is the most ideal approach among the five typesof vaginoplasty investigated in this trial.
