胆道镜联合钬激光治疗肝外胆管残余结石的术后护理

目的探讨胆道镜联合钬激光治疗肝外胆管残余结石的术后护理。方法46例均采用胆道镜联合钬激光取石术,术后采取卧位、饮食指导、引流管护理并观察有无并发症发生等措施。结果46例均碎石取石成功。取石时间15～50min,平均30min。术中无胆管损伤及结石残留。46例术后2d胆道造影显示无残余结石后拔除引流管。其中38例获得随访,随访时间6～12月,无残余结石及胆道狭窄发生。结论胆道镜联合钬激光取石术是胆总管残余结石治疗的安全有效方法,但术后的较好护理也是保证手术成功的关键。

Nursing Care after The Operation of Extrahepatic Billiary Residual Calculus by Choledochoscope and Holmium Laser

Objective] To explore the nursing care after the operation of extrahepatic billiary residual calculus by choledochoscope and holmium laser. Methods] A total of 46 cases experienced treatments of choledochoseope and holmium laser; horizontal position, diet guidance, drainage duct care and observation of related complications were conducted. Results] All 46 lithotripsy procedures were completed successfully within 15 -50 minutes （ mean value 30 minutes）. There were no bile duct injury and residual calculus during operations. After 2 days, drainage ducts were removed based on cholangiography without residual lithiasis. Among which, 38 were followed up within 6 - 12 months, no residual lithiasis and bile duct stricture occurred. Conclusion ] Choledochoscope and holmium laser is safe and effective in treatment of extrahepatic billiary residual calculus, well postoperative nursing care is also the key to ensure the success of operation.
