原发性肌肉骨关节肿瘤性(样)病变的MRI征象探讨

探讨原发性肌肉骨关节肿瘤及肿瘤样病变MR征象及其诊断价值。对52例原发性肌肉骨关节肿瘤及肿瘤样病变行MR常规扫描并经病理证实。结果显示：88.5%的肿瘤边缘可见低信号带；明显的瘤周水肿（25%）仅见于恶性肿瘤及呈侵蚀性生长的良性证实。结果显示：88.5%的肿瘤边缘可见低信号带；明显的瘤周水肿（25%）仅见于恶性肿瘤及呈侵蚀性生长的良性病变，邻近结构受侵征象在恶性肿瘤中的出现率（69.6%)明显高于良性（17.2%)；信号分布均匀性程度在良，恶性病变中有较高的重叠性，提示：综合分析肿瘤MR征象对鉴别原发性肌肉骨关节肿瘤及肿瘤样病变的良，恶性有价值。

MR Signs of the Primary Musculoskeletal Neoplasms and Neoplasm Mimickers

The diagnostic value of the MR signs of the primary musculoskeletal neoplasms and neoplasm mimickers was studied. MR scanning was performed on 52 cases of primary musculoskeletal neoplasms and neoplasm mimickers proved by surgery and pathology. Statistical analysis was used to test for the differences in 4 MR signs between the benign and malignant tumors. The results showed: (1) Low signal belt (LSB) of perineoplasm was observed in 46 of 52 neoplasms (88.5 %); (2) Marked Perineoplastic edema was presented in 13 of 52 cases (25 %), including 8 malignant lesions and 5 benign ones with eroside features; (3) The incidence of the invasive signs of the adjacent structures in malignant neoplasms (69 6 %) was significantly higher than in benign ones (17 2 %); (4) The rate of homogeneous signal intensity and heterogeneous signal intensity showed in benign and malignant lesions were 20 7 %/79 3 %, 26 1 %/73 9 %, respectively. Obvious overlap existed between benign and malignant lesions. It was concluded that the analysis of the MR signs, the patterns of tumor's border, degree of perineoplastic edema and involving bone or neurovascular structures had some significant values in differentiating benign from malignant primary musculoskeletal neoplasms.
