尿酸和C-反应蛋白与2型糖尿病高血压和肾病的相关性

摘    要:目的探讨高尿酸血症和C-反应蛋白(CRP)与T2DM合并高血压、肾病的关系。方法选择132例T2DM患者,分成单纯DM组、DM合并高血压组和DM肾病组,测定血UA、CRP、Cr、BUN、空腹血糖(FBG)、糖化血红蛋白(HbAlc)、空腹胰岛素(FINS)和血脂水平。结果DMEH组和DN组血uA和CRP水平显著高于健康对照组(P〈0.05,P〈0.01);DN组血Cr水平明显高于健康对照组和单纯DM组(P〈0.001,P〈0.01);多元回归分析显示,T2DM患者血UA与DBP、TG、Cr呈显著正相关,而与HDL—C呈显著负相关;血UA与FINS呈正相关;CRP与FPG、UAER和Cr呈显著正相关(P〈0.05)。结论高尿酸和CRP可能参与T2DM高血压及肾病的发生发展过程,动态监测可作为DMEH和DN的预告因子。

Abstract:Objective To study the relation between hyperuricemia, C-reactive protein (CRP) and type 2 diabetes mellitus (T2DM) with hypertension and nephropathy. Methods A total of 132 of T2DM patients were selected and divided into three groups. Patients in group 1 had no combined disease, patients in group 2 had combined high blood pressure, and patients in group 3 had combined nephropathy. Levels of blood uric acid (UA), CRP, creatinine (Cr), blood urea nitrogen, fasting glucose, glycosylated hemoglobin, fasting insulin and blood-fat were measured. Results The level of UA was significantly higher in all experimental groups than in control group (P〈0.05, P〈0.01). The level of Cr was significantly higher in simple DM group than in control and T2DM groups (P〈0.001, P〈0.01). Multiple regression analysis showed that UA in the T2DM group was positively correlated with diabetic blood pressure (DBP), TG and Cr, but negatively correlated with HDL-C. UA was positively correlated with FINS. CRP in the T2DM group was positively correlated with FPG, UAER and Cr. Conclusion Diabetic high blood UA and CRP may be involved in the occurrence and development of high blood pressure and nephropathy. Their dynamic monitoring can predicate DMEH and DN.
