血清PSA、F-PSA及F-PSA/PSA比值对前列腺癌的诊断价值

摘    要:目的探讨T-PSA、F-PSA、F-PSA/T-PSA比值对前列腺癌的诊断价值。方法采用全自动电化学发光免疫分析仪(ECLIA)检测86例健康男性、46例前列腺增生和48例前列腺癌患者血清T-PSA和F-PSA,计算F-PSA/T-PSA比值,并进行统计分析。结果前列腺癌患者血清T-PSA明显高于前列腺增生组及正常对照组(P〈0.01),F-PSA/T-PSA比值明显低于前列腺增生组及正常对照组(P〈0.01),T-PSA与前列腺癌分期存在正相关性(r=0.786,P〈0.001),Ⅲ-Ⅳ期患者的T-PSA明显高于Ⅰ-Ⅱ期,而F-PSA/T-PSA比值与前列腺癌分期无相关性(P〉0.05)。结论T-PSA和F-PSA对前列腺癌有较高的诊断价值,F-PSA/T-PSA比值可用于前列腺癌和前列腺增生的鉴别诊断。

Abstract:Objective To investigate the diagnostic value of serum total prostate specific antigen (T-PSA), free PSA (F-PSA) and their ratio for prostatic cancer. Methods Serum T-PSA and F-PSA levels were measured by ECLIA, and F-PSA/T-PSA ratio was calculated in 48 patients with prostatic cancer, 46 patients with benign prostate hyperplasia (BPH) and 86 controls, respectively. Results The serum T-PSA level was significantly higher in patients with prostatic cancer than in those with BPH and in healthy individuals(P〈0.01), whereas the level of F-PSA/T-PSA was much lower in patients with prostatic cancer than in those with BPH and in healthy individuals(P〈0.01). The level of T-PSA in prostatic cancer patients was positively correlated with prostatic cancer stages(r=0.786, P〈0.001) and significantly higher in patients with Ⅲ - Ⅳ stage of prostatic carcinoma than in those with Ⅰ - Ⅱ stage of prostatic carcinoma, while the F-PSA/T-PSA ratio was not correlated with prostatic carcinoma stage(P〉0.05). Conclusion T-PSA and F-PSA have a great diagnostic value for prostatic cancer. F-PSA/T-PSA ratio can be used as an indicator in differential diagnosis of prostate cancer and BPH.
