不同原因致桡神经损伤的显微外科修复

目的 探讨由不同原因致桡神经损伤的治疗方法扣临床效果。方法 临床治疗不同原因致桡神经损伤37例，对于新鲜的桡神经损伤：神经缺损不多，张力不大常采用端端缝合；完全断裂行神经外膜法缝合；部分断裂行神经束膜缝合法或神经束组缝合法。陈旧性桡神经损伤：缺损在其直径4倍以内，张力不大，采用神经外膜法端端缝合；缺损超出其直径4倍以上，张力较大，采用皮神经移植；缺损大于5cm则采用带血管的神经移植；神经连续性存在，采用神经外膜松解；神经断裂位于神经肌肉接头处，采用神经植入法修复。结果 按中华医学会手外科学会上肢部分功能评定试用标准评定，优23例，良7例，中7例，优良率81．8％。结论 采用适宜的显微外科技术方法修复桡神经损伤可以获得较好的功能恢复。

Microsurgical repair of radial nerve injury caused by different reason

Objective To probe the treatment outcome of radial nerve injuries caused by different reason.Methods We treated37cases with radial nerve injuries.According to the type,the different surgical techniques of microsurgery had been used,such as side-to-side neurorrhaphy in acute injury of radial nerve and nerve transfer in longer defect of radial nerve.Results According to the standard of the Hand Surgery Association in Chinese Medical Association,excellent function occurred in23cases and good function occurred in7cases,the rate of superior was81.8%.Conclusion It is feasible to repair radial nerve injury with microsurgical techniques,which could abtain good function.
