近5年中医外治法治疗慢性腹泻研究述评

慢性腹泻(chronic diarrheal disease,CDD)的发病与胃、脾、肝、肾关系密切,且互相影响。其病因病机以虚实夹杂为主,其中虚者为脾胃虚弱,运化失司,水液停滞;实者以肝气郁滞、外感风寒湿、饮食瘀血内停影响脾胃功能。慢性腹泻的中医外治法包括针刺、推拿、艾灸、穴位注射及穴位埋线治疗等。目前,仍存在以下问题:第一,样本纳入无统一辨证标准,且收集样本量小,缺乏说服力;第二,文献中推拿对治疗成年人慢性腹泻研究较少;第三,针灸选穴和治疗时间上未形成规范的体系,并且缺少对各年龄段CDD患者的分层研究。

A Review of Chronic Diarrhea by External Treatment of TCM in the Past Five Years

The occurrence of chronic diarrhea disease(CDD)is closely related to the stomach,spleen,liver and kidney,and affects each other.The etiology and pathogenesis of CDD are mainly the mixture of deficiency and excess.The deficiency refers to the weakness of the spleen and stomach,dysfunction of transportation and transformation,and the stagnation of water,while the excess is stagnation of liver qi,exogenous wind and cold and dampness,and stagnation of blood stasis and food retention,which affects the function of the spleen and stomach.The TCM treatment of CDD includes acupuncture,massage,moxibustion,acupoint injection,acupoint catgut embedding,etc.At present,there are still the following problems:(1)There is no unified standard of syndrome differentiation when selecting patients,and the collection of samples is small,lack of persuasion;(2)There are few studies on massage in the literature;(3)There is no standard system in acupuncture acupoint selection and treatment time,and there is no stratified study on CDD patients of all ages.
