老龄化背景下常见老年慢性病患者次均住院费用影响因素分析

目的探讨常见老年慢性病患者次均住院费用及其影响因素。方法通过电子病历系统提取2018年3月至2019年2月泉州某三甲医院高血压、冠心病、脑血管疾病、2型糖尿病、恶性肿瘤和慢性阻塞性肺病的老年慢性病患者21 824例的信息,用多元线性回归模型,分析患者次均住院费用的影响因素。结果老年慢性病患者次均住院费用较高(平均10 036.4元),入院方式、慢性病类型、支付方式、住院天数、住院次数是影响老年慢性病患者次均住院费用的主要因素。结论老年慢性病患病率高、卫生资源消耗多、住院费用高,次均住院费用的影响因素复杂。应积极开展早期预防,有效合理控制老年慢性病的住院费用;强化政府职能,保障老年慢性病住院治疗和费用,防止因病返贫。

Analysis on influencing factors of mean hospitalization cost per time of common elderly patients with chronic diseases in the background of population aging

Objective To explore the mean hospitalization cost per time and its influencing factors in common elderly patients with chronic diseases. Methods Based on the electronic medical record system, the medical data of 21 824 elderly patients with chronic diseases diagnosed with high blood pressure, coronary heart disease, cerebrovascular disease, type 2 diabetes, malignant tumor and chronic obstructive pulmonary disease from March, 2018 to February, 2019 in a third-grade class-A hospital in Quanzhou City was extracted and the influencing factors of mean hospitalization cost per time of the patients were analyzed by using multiple linear regression model. Results The maen hospitalization cost per time of elderly patients with chronic diseases was at higher level(mean=10 036.4 Yuan).The main influencing factors were the way of hospitalization, type of chronic diseases,payment method,days and frequency of hospitalization. Conclusion The influencing factors of mean hospitalization cost per time were complicated, such as higher prevalence of elderly patients with chronic diseases, higher consumption of health resources and the higher hospitalization cost. Early prevention should be actively carried out, with reasonably controlling the hospitalization cast of elderly patients with chronic diseases. The function of government should be strengthened,the treatment and medical cost of elderly hospitalization patients with chronic diseases should be guaranteed for preventing the poverty caused by the diseases.
