肝外伤患者术后营养支持的研究

目的 探讨肝外伤肝切除术后应用早期肠内营养的价值。方法 将26例肝外伤实行肝切除术的患者随机分成两组，手术后分别接受肠内营养和肠外营养1周，观察两种营养支持方法对患者的基础营养状况、肝功能恢复情况、胃肠功能恢复情况、并发症发生率的影响以及营养支持的费用的异同。结果 术后早期使用肠内营养支持组其术后营养状况与肠外营养组比较差异无统计学意义（P〉0．05），但能促进肝功能的恢复，促进胃肠功能的恢复，而且更加经济，两组比较差异均有统计学意义（P〈0．05）。结论 肝外伤肝切除术后应用早期肠内营养，是一种经济、有效的营养支持手段。

Postoperative nutritional support of the patients with liver trauma

Objective To evaluate early enteral nutrition of patients after hepatectomy of liver trauma.Methods Thirty-eight patients were randomized into two groups to respectively receive enteral or parenteral nutritional support beginning at the first day post-operation for one week.Patients' nutritional status,liver function,gut function,complication rate and expense were recorded and compared.Results Patients' post-operative nutritional status between early enteral nutritional group and parenteral nutritional group had no difference (P>0.05).Enteral nutrition could improve the recovery of liver function and gut function,and the expense were less than that of parenteral nutrition (P<0.05).Conclusion Early enteral nutrition was an effective and economical nutrition support method after hepatectomy of liver trauma.
