腕关节类风湿性关节炎的MRI早期诊断

目的分析早期类风湿性腕关节炎的磁共振成像（MRI）表现,以期提高早期诊断水平。方法回顾分析35例早期类风湿性关节炎患者腕关节的MR扫描结果 ,均经临床及生化检查证实,其中男性10例,女性25例,年龄19-67岁,平均44.5岁,采用GE1.5TMR扫描仪,共扫描70个腕关节。结果 60个腕关节出现不同程度的信号异常,其中双侧受累25个,单侧受累10个,滑膜受累表现为滑膜增厚,T2加权像滑膜信号增高,7例腕间关节少量积液,9例腕关节构成骨出现骨质缺损、骨髓水肿等异常改变。结论 MR成像是腕关节类风湿性关节炎早期诊断的重要和可靠的检查手段,特别是出现双侧腕关节受累,滑膜增厚、强化及腕骨信号异常等,应考虑本病。

MRI in the early diagnosis of wrist rheumatoid arthritis

Objective To analyze the signs of magnetic resonance imaging （MRI） in the diagnosis of early rheumatoid arthritis （RA）.Methods 35 patients with RA, 10 males, 25 females, aged 19-67years were performed GE 1.5T MR scanner.Results Among 60 wrists with RA, 25 patients had bilateral synovial enhancement or thickening as criterion; 10 patients had unilateral synovial enhancement or thickening.Conclusion MRI is one of the most reliable diagnostic methods for RA and it is extremely useful in the early diagnosis. The early diagnosis of RA was highly suggested when bilateral synovial enhancement and thickening was presented.
