两种不同方法治疗老年股骨颈骨折的疗效比较

目的观察对比人工股骨头置换术、人工全髋置换术治疗股骨颈骨折的临床效果，以指导临床选择应用。方法将2008年1月～2011年6月在我院住院治疗行人工股骨头置换术的老年股骨颈骨折患者26例设为A组，另选择同期行全髋关节置换术的26例患者作为对照组，设为B组，对比分析两组的手术时间、术中出血量、Harris评分、手术并发症等情况。结果与B组比较，A组手术时间短、术中出血量少。但A组Harris评分明显低于B组，且A组并发症多于B组（P〈0．01）。结论人工股骨头置换术、人工全髋置换术治疗股骨颈骨折各具优缺点．临床上府视患者具体情况加以选择．

Comparison of two different methods in treatment of femoral neck fracture in the elderly

Objective To observe the clinical effect of artificial femoral head replacement surgery, total hip arthro- plasty for femoral neck fracture, and further to guide clinical select applications. Methods From January 2008 to June 2011 in our hospital, 26 cases elderly patients with femoral neck fracture as the group A, and the other under- went total hip arthroplasty 26 patients as control group, the two groups of operative time, blood loss, Harris score, sur- gical complications were compared. Results Compared with group B, A group of operative time, less blood loss. Group A Harris scores were significantly lower in group B, and complications of group A was much higher than group B（P 〈 0.01 ）. Conclusion Artificial femoral head arthroplasty, total hip arthroplasty for femoral neck fractures each with ad- vantages and disadvantages, clinicaL should be selected depending on the patient specific circumstances.
