逆行撕除联合带结膜瓣自体角膜缘干细胞移植及5-氟尿嘧啶治疗翼状胬肉疗效观察

目的观察逆行撕除联合带结膜瓣自体角膜缘干细胞移植及5-氟尿嘧啶治疗翼状胬肉的临床疗效。方法在我院2005年1月～2008年12月期间收治的135例（142眼）患者，随机分为观察组65例，行翼状胬肉逆行撕除联合带蒂结膜瓣移植及联合应用5-氟尿嘧啶治疗；对照组70例，行单纯翼状胬肉切除术，观察术后愈合情况、并发症，并对2年后两种术式复发率进行对比分析。结果2年后随访观察组复发率3．08％，对照组复发率22．86％。组间复发率比较差异具有统计学意义（P〈0．05）。结论逆行撕除翼状胬肉联合自体角膜缘干细胞移植及5-氟尿嘧啶治疗翼状胬肉．方法简便．效果满意，复发率低，无并发症．值得临床推广。

Observation about the clinical effect of peeling-off in a reverse direction combined with autologous corneal limbal stem cell transplantation with conjunctival flap and 5-fluorouracil on pterygium

Objective To observe the clinical effect of peeling-off in a reverse direction combined with autologous corneal limbal stem cell transplantation with conjunetival flap and 5- fluorouracil on pterygium. Methods From Jan- uary 2005 to December 2008 135 cases （142 eyes） were selected, they were randomly divided into observation group （65 cases） and comparison group （70 cases）. The observation group with line pterygium retrograde tore in addition to joint pedicle conjunetival flap transplantation and the combined use of 5- fluorouracil treatment while the comparison group with simple pterygium resection. The postoperative healing, complications, and the recurrence rate after 2 years, two kinds of operation were compared and analyzed. Results After two years, the recurrence rate of observation group was 3.08%, the recurrence rate of the comparison group was 22.86%, had significant difference （P 〈 0.05）. Conclusion Peeling-off in a reverse direction combined with autologous corneal limbal stem cell transplantation with conjunctival flap and 5- fluorouracil on pterygium are simple and effectual with few complications and low recurrence, worthwhile for spreading in clinical practice.
