胰岛素泵用于糖尿病围手术期28例

目的探讨胰岛素泵在糖尿病围手术期中的应用价值。方法将医院需手术的2型糖尿患者60例随机分为2组,对照组使用常规胰岛素强化治疗,观察组使用胰岛素泵强化治疗。观察两组血糖、胰岛素用量、低血糖发生率、血糖达标时间、术后抗生素使用时间、伤口感染发生率。结果治疗后两组血糖均下降,但在餐后血糖控制方面,观察组明显优于对照组(P<0.05);观察组血糖达标时间、抗生素使用时间明显缩短(P<0.05),胰岛素用量明显减少(P<0.05),低血糖发生率及切口感染率均明显降低(P<0.05)。结论围手术期糖尿病患者使用胰岛素泵治疗,疗效明显优于常规胰岛素强化治疗。

Application of Continuous Subcutaneous Insulin Infusion Pump in 28 Cases of Perioperative Treatment of Diabetes Mellitus

Objective To evaluate the therapeutic role of continuous subcutaneous insulin infusion pump（CSII） used in perioperative pe- riod of type 2 diabetes mellitus（T2DM）. Methods Sixty cases of T2DM needing operation in this hospital were randomly assigned to two groups. The control group was treated by the routine intensive insulin, while the observation group was given intensive CSII. Blood glucose, dosage of insulin, hypoglycemia incidence, time reaching the standard of blood glucose, time of postoperative antibiotic use, post- operative wound infection rate were observed and compared between the two groups. Results The blood glucose in both two groups was decreased. But in the aspect of controlling postprandial blood glucose, the observation group was significantly superior to the control group（P 〈 0. 05）. The time reaching the standard of blood glucose and the time of postoperative antibiotic use in the observation group were obviously shortened , the dosage of insulin was obviously decreased, the hypoglycemia incidence and the infection rate of operation incisions all were significantly reduced（ P 〈0. 05）. Conclusion CSII is remarkably superior to the routine intensive insulin therapy in the perioperative treatment of T2DM patients.
