摘    要:目的探讨胰岛素强化治疗对T2DM患者尿单核细胞趋化蛋白1(UMCP-1)排泄的影响及其意义。方法30例T2DM患者进行为期2周的胰岛素强化治疗,比较治疗前后DM相关代谢指标及UMCP-1排泄的变化。结果T2DM患者的FPG、2hPG、HbA1c和uMcP_1/尿肌酐(UCr)比值明显高于正常对照组;T2DM患者强化治疗后UMCP-1/ucr比值明显降低(P〈0.01);DM患者UMCP-1/ucr比值与尿白蛋白/UCr呈正相关(P均〈0.01)。结论短期胰岛素强化治疗可降低DM患者UMCP-1排泄,减轻糖尿病高血糖状态下肾组织局部增强的炎症反应。

Effect of intensive insulin therapy on urinary monocyte chemoattractant protein-1 excretion in patients with type 2 diabetes mellitus

Abstract:Objective To investigate the effect of intensive insulin therapy (IIT) on urinary monocyte chemoattractant protein-1 (UMCP-1) excretion and explore its significance in the patients with type 2 diabetes mellitus. Methods We examined the UMCP 1, urinary creatinine and urinary albumin (UAlb) level in 30 patients with T2DM and 20 normal subjects(NC). All diabetic patients were given intensive insulin therapy for 2 weeks. After 2 weeks treatment, the levels of UMCP-1, UCr and UAIb were measured once again. Results Not only the FBG,2hPG and HbA1, but also the ratio of UMCP- 1/UCr increased significantly in T2DM versus NC group (P〈0. 01 or P〈0.05). They all significantly decreased after 2 weeks of intensive insulin therapy, with the significantly decreased ratio of UMCP-1/UCr after versus before IIT of (104.80±51.88) vs (184.51±110. 60) pg·mg^-1·Cr^-1 ,P〈0.01. The ratio of UMCP-1/UCr was positively correlated with UAlb/UCr(r=0. 452, P〈0. 01). Conclusions Shortterm intensive insulin therapy decreases the UMCP-1 excretion and lessens the inflammatory reaction of renal tissue under hyperglycemia state.
