晚期非小细胞肺癌NP方案同步放化疗疗效观察

目的对比观察NP方案（益诺加顺铂）同步放化疗和诱导化疗加放疗,对局部晚期非小细胞肺癌的疗效。方法56例局部晚期非小细胞肺癌患者分为同步组和诱导组,每组各28例。同步组：盖诺25mg／m2加入生理盐水100ml,静脉滴注,第1天,第8天;顺铂30mg／m2静脉滴注,第1-3天。28d为1周期,连续6周期,化疗第2天开始放射治疗,放疗采用常规分割,2Gy／次,5次／周,照射范围包括原发病灶、纵隔淋巴引流区和转移淋巴结。原发灶总剂量60Gy,转移淋巴结剂量40Gy。诱导组的化疗方案及放疗方法和同步组一致,经2周期化疗后,开始行放射治疗,放疗期间停止化疗,放疗结束詹,再继续化疗4个周期。结果随访1年以上,同步组完全缓解6例,部分缓解15例,有效率为75．0％,中位生存期为18个月;诱导组：完全缓解4例,部分缓解13例,有效率为60．7％,中位生存期为13个月（P〉0．05）。结论NP方案同步放化疗可提高局部晚期非小细胞肺癌的生存率,为较有效的治疗方法。

The curative effect obseravation of NP program simultaneous radiotherophy and chemotheropy

Objective To observe the therapeutic effect of chemotherapy with NP regimen and radiotherapy for NSCLC. Methods 56patients with NSCLC were randomized into 2 groups, Combination group and Seduce group, 28 patients in each group. In combination group： the patients received radiotherapy plus chemotherapy with NP regimen 6 cycles ; In Seduce group, chemotherapy was given 2 cycles, radiotherapy followed, during radiotherapy, with no chemotherapyg. After radiotherapy, chemotherapy was given 4 cycles with NP regimen. Results The results showed that the CR ＋ PR in combination group was 75.0 %, in Seduce group 60.7 % （P 〉 0.05 ）. Conclusion The effectiveness of radiotherapy combined with chemotherapyg in NP regimen is superior to that of radiotherapy with seduce ehemotherapyg.
