腹腔镜技术在局部复发性直肠癌再手术中的应用价值

直肠癌术后局部复发率约为10%。大部分病人在肿瘤复发后出现较严重症状,影响病人生命质量和预后。尽管再次手术的难度较大,但其仍然是治疗直肠癌局部复发的主要方式。目前再次手术的主要方式有经腹前切除、联合盆腔脏器切除以及联合骶骨切除等,以开放手术为主。近年来随着腹腔镜技术的广泛开展,国内外研究者尝试采用腹腔镜技术进行局部复发性直肠癌的再手术治疗,近期疗效良好。笔者综合分析国内外相关研究进展,系统阐述腹腔镜技术运用于局部复发性直肠癌的现状,探讨其临床应用前景和推广价值。

Application value of laparoscopic technology in reoperation of locally recurrent rectal cancer

The incidence of locally recurrent rectal cancer is about 10%.Most patients have serious symptoms after tumor recurrence,which seriously affects the quality of life and prognosis of patients.Although it is more difficult to perform reoperation,it is still the main method to treat local recurrence of rectal cancer.At present,the main methods of reoperation include transabdominal anterior resection,combined pelvic organ resection,combined pelvic organ resection,and sacral resection.Open surgery is the main method.In recent years,with the extensive development of laparoscopic technology,laparoscopic techniques have been tried at home and abroad to perform reoperations for locally recurrent rectal cancer,showing good short-term results.The authors systematically introduce the application of laparoscopic technology in the reoperation of locally recurrent rectal ncer based on relevant research advances at home and abroad,in order to explore its clinical application prospects and promotion value.
