慢性心力衰竭患者再入院风险因素的Meta分析

目的:通过Meta分析明确慢性心力衰竭患者再入院的相关风险因素。方法:计算机检索PubMed、Cochrane Library、Web of Science、Embase、SinoMed、CNKI、万方数据库和维普数据库,检索时限为建库至2019年12月31日。采用RevMan 5.3软件进行Meta分析。结果:共纳入19篇文献,纳入患者185545例,包括34项相关风险因素。有统计学意义的风险因素包括抑郁情绪(OR=3.87,95%CI:2.87~5.23,P<0.01)、独居(OR=1.12,95%CI:1.01~1.24,P=0.03)、合并高血脂或糖尿病(OR=1.16,95%CI:1.14~1.19,P<0.01)、高盐饮食(OR=3.36,95%CI:2.60~4.35,P<0.01)、肾功能下降(OR=2.00,95%CI:1.95~2.04,P<0.01)、射血分数下降性心力衰竭(OR=2.71,95%CI:1.93~3.73,P<0.01)、贫血(OR=1.43,95%CI:1.27~1.60,P<0.01)、感染(OR=1.12,95%CI:1.00~1.25,P=0.04)。结论:抑郁情绪、独居、合并高血脂或糖尿病、高盐饮食、肾功能下降、射血分数下降性心力衰竭、贫血、感染是慢性心力衰竭患者再入院的相关风险因素。

Risk factors for readmission in patients with chronic heart failure: a Meta-analysis

Objective To explore the relevant risk factors for readmission of patients with chronic heart failure by Meta-analysis.Methods We searched the PubMed,Cochrane Library,Web of Science,Embase,SinoMed,China National Knowledge Infrastructure(CNKI),Wanfang Data and VIP Database by computer,and the search time limit was from the establishment of the database to December 31,2019.RevMan 5.3 software was used for Meta-analysis.Results A total of 19 articles were included,including 185545 patients and 34 related risk factors.Statistically significant risk factors included depressionOR=3.87,95%CI(2.87-5.23),P<0.01],living aloneOR=1.12,95%CI(1.01-1.24),P=0.03],combined with hyperlipidemia and diabetesOR=1.16,95%CI(1.14-1.19),P<0.01],high-salt dietOR=3.36,95%CI(2.60-4.35),P<0.01],decreased renal functionOR=2.00,95%CI(1.95-2.04),P<0.01],heart failure with reduced ejection fractionOR=2.71,95%CI(1.93-3.73),P<0.01],anemiaOR=1.43,95%CI(1.27-1.60),P<0.01]and infectionOR=1.12,95%CI(1.00-1.25),P=0.04].Conclusions Depression,living alone,combined with hyperlipidemia and diabetes,high-salt diet,decreased renal function,heart failure with reduced ejection fraction,anemia and infection are related risk factors for readmission in patients with chronic heart failure.
