胰岛素对大鼠急性脊髓损伤保护作用的实验研究

摘    要:目的:探讨胰岛素对大鼠急性脊髓损伤的保护作用及其效果。方法:采用改良Allen WD法建立大鼠急性脊髓损伤模型。60只SD大鼠随机分为损伤对照组和胰岛素治疗组,术后8h,1,3,7,14d检测大鼠脊髓损伤后运动诱发电位(MEP)的潜伏期及波幅;采用原位末端标记(TUNEL)观察脊髓细胞凋亡;采用激光多普勒血流仪(LDF)监测了胰岛素对伤段脊髓SCBF量的影响。结果:术后各时相胰岛素组MEP潜伏期与对照组相比无明显差异,MEP波幅较对照组明显增高(P<0.05);胰岛素治疗组TUNEL阳性细胞的数目明显少于损伤对照组(P<0.05);两组脊髓血流量(SCBF)均明显下降,但对照组下降更明显。结论:初步研究表明胰岛素能改善脊髓血流量、抑制神经细胞的凋亡、减轻神经功能缺失,对急性脊髓损伤有保护作用。

Abstract:Objective:To study the protective effect of insulin to acute spinal cord injury in rats.Method:Acute spinal cord inju- ry model in rats was built by modified Allen WD.Sixty healthy adult Sprague-Dawley rats were randomly divided into two groups:con- trol group and insulin group.The latency and amplitude of MEP(Motion Evoked Potential) were analyzed at 8h,1d,3d,7d,14d postoperatively (n=6 in each time point) were analyzed.Apoptosis neurons were labeled with TUNEL dyeing.The spinal cord blood flow (SCBF) was recorded by laser-Doppler flowmetry.Result:There were no changes in the latencies of MEPs in both groups,and the mean amplitudes of MEPs were higher in insulin groups than in control groups.The number of TUNEL staining positive cells in the in- sulin group was less than in control group(P<0.05).And SCBF decreased in both groups,especially in control group.Conclusion: The study indicated that insulin could improve SCBF,inhibit neurocyte apoptosis,palliate loss of neural function,which might protect injured spinal cord.
