胃癌患者血清及组织中组织蛋白酶-D、谷胱甘肽-S-转移酶活性测定的临床意义

摘    要:目的 探讨组织蛋白酶 - D(Cath- D)、谷胱甘肽 - S-转移酶 (GST)活性变化与胃癌发生、发展及预后的关系。 方法 检测胃癌、单纯性胃炎、胃炎伴癌前病变患者血清、癌组织、癌旁组织、胃窦粘膜中 Cath- D、GST的活性和手术前后血清 GST活性。 结果  (1)胃癌组的 Cath- D、GST的活性显著高于单纯性胃炎组及胃炎伴癌前病变组 (P<0 .0 1) ,胃炎伴癌前病变组 GST的活性显著高于单纯性胃炎组 (P<0 .0 5 ) ,Cath- D活性与单纯性胃炎组比较差异无显著性 (P>0 .0 5 )。 (2 )胃癌组织 Cath- D、GST的活性显著高于癌旁组织 (P<0 .0 5 )、癌旁组织中Cath- D、GST的活性显著高于对照组织 (P<0 .0 5 )。 (3)伴有淋巴结转移的胃癌组织中 Cath- D活性显著高于不伴有淋巴结转移的 (P<0 .0 5 ) ,GST的活性在伴有与不伴有淋巴结转移者中差异无显著性 (P>0 .0 5 )。 (4) Cath- D的活性与按预后好坏的组织学分型 (P<0 .0 1)、按 TNM的分期 (P<0 .0 1)等病理指标有密切相关 ,而 GST的活性与病理指标无显著相关 (P>0 .0 5 )。 (5 )胃癌手术后 10天血清中 GST的活性明显低于手术前 (P<0 .0 1)。 结论 Cath- D、GST活性联合检测可用于胃癌高危人群筛选、监测癌前病变进展及预测肿瘤复发

Abstract:Objective To study the relationship between activity change of Cath|D and GST and the incidence,development, prognosis of gastric carcinoma. Methods The activity of Cath|D and GST in serum,carcinoma tissue, paracarcinoma tissue, normal antral gastric tissue were assayed in gastric carcinoma and chronic simple gastritis and chronic gastritis complicated with precarcinous lesion. The activities of GST in serum of gastric carcinoma were also compared before|and after operations. Results (1)The activity of Cath|D and GST in tissue of gastric carcinomas were higher than control groups(P<0.01) and chronic gastritis complicated with precarcinous lesion(P<0.01). The activity of Cath|D and GST in tissue of chronic gastritis complicated with precarcinous lesion were higher than control groups, only GST in tissue was significantly(P<0 05). (2)The activity of Cath|D and GST with tissue of gastric carcinomas were higher than in near tissue(P<0 05). The two enzyme activities in near tissue were higher than those in normal tissue(P<0 05). (3)The activity of Cath|D and GST in node|positive gastric carcinomas was higher than the node|negative gastric carcinomas. (4)The activity of Cath|D was correlated with histological typing according to prognosis(P<0 01) and TNM stage(P<0 01). (5) The activity of GST in serum of patients were decreased after operation(P<0 01). Conclusion The assay of Cath|D and GST activities are useful in screening of the high|risk group developing gastric carcinomas, mornitoring the progress of precarcinous lesions and predicting the relapse of tumors.;
