多黏菌素 B多途径给药联合替加环素治疗 1例泛耐药鲍曼不动杆菌肺部感染的疗效分析

摘    要:目的探讨泛耐药鲍曼不动杆菌肺部感染病人抗感染治疗的有效性和安全性。方法回顾性分析南通大学附属医院 2018年 2—3月收治的 1例泛耐药鲍曼不动杆菌肺部感染病人的治疗过程,结合病人临床症状和相关检查进行有效性和安全性评价。结果多黏菌素 B(静脉滴注 +雾化吸入)联合替加环素治疗 10 d后,病人临床症状好转;替加环素用药 8d后,病人门冬氨酸氨基转移酶(AST)和丙氨酸氨基转移酶(ALT)分别高于正常值 5.73倍和 3.85倍。结论多黏菌素 B多途径给药模式联合替加环素对泛耐药鲍曼不动杆菌肺部感染具有良好的抗菌效果;其安全性有待进一步评价。

Abstract:Objective To investigate the efficacy and safety of anti-infective therapy in patients of pulmonary infection with exten-sively drug resistant Acinetobacter baumannii.Methods The treatment process of a patient who were admitted to the Affiliated Hospi-tal of Nantong University from February to March 2018 of pulmonary infection with pan-resistant A. baumannii were retrospectively an-alyzed, and the efficacy and safety were evaluated in combination with patient''s clinical symptoms and related examinations.Results Ten days after treatment with polycolistin B (intravenous + aerosolized) and tigacycline, the patient''s clinical symptoms was improved.Eight days after treatment with tigacycline, patient''s AST and ALT were 5.73 and 3.85 times higher than normal values, respectively.Conclusions It has a good antibacterial effect on pulmonary infection of pan-drug resistantce A. baumannii with Multiroute adminis-tration of polycolistin B combined with tegacycline, and its safety needs further evaluation.
