Abstract:Objective To explore the correlation between salivary oxidative stress and immune indexes and the severity of dental caries in children.Methods From March 2019 to March 2021,107 children with caries(caries group)were selected from the Children's Department of Stomatology,Stomatological Hospital of Xi'an Jiaotong University.According to the number of missing filling surfaces(DMFT)of deciduous teeth,they were divided into low caries subgroup(0-4,42 cases)and high caries subgroup(≥5,65 cases),and 83 healthy children were selected as the healthy control group.Saliva of all subjects was collected on the day of enrollment,and malondialdehyde(MDA),total antioxidant capacity(TAC),catalase(CAT),superoxide dismutase(SOD),secretory immunoglobulin A(sIgA),immunoglobulin G(IgG),IgA and IgM in saliva were detected.Multiple linear regression was used to analyze the correlation between salivary MDA,TAC,cat,SOD,SIgA,IgG,IgA,IgM and DMFT.Results The levels of MDA in saliva of caries group were lower than those of healthy control group(t/P=14.678/0.000),and the levels of TAC,SOD,cat,SIgA,IgG,IgA and IgM were higher than those of healthy control group(t/P=16.818/0.000,9.124/0.000,10.613/0.000,11.005/0.000,2.339/0.017,2.486/0.014 and 4.006/0.000).The levels of MDA in saliva of high caries subgroup were lower than those of low caries subgroup(t/P=13.589/0.000),and the levels of TAC,SOD,cat,SIgA,IgG,IgA and IgM were higher than those of low caries subgroup(t/P=11.434/0.000,16.734/0.000,13.287/0.000,12.961/0.000,13.538/0.000,3.906/0.000 and 29.029/0.000).Multiple linear regression analysis showed that salivary MDA was negatively correlated with DMFT(β/P=-0.216/0.000),TAC,cat,SOD,SIgA,IgG,IgA and IgM were positively correlated with DMFT(β/P=0.303/0.000、0.471/0.000、0.186/0.019、0.678/0.000、0.209/0.040、0.374/0.036、0.413/0.023)。The area under the curve(AUC)of salivary MDA,TAC and SIgA to identify the severity of caries was 0.713,0.744 and 0.730(P<0.01),and the AUC of the combination of the three was 0.911,which was higher than that of single index diagnosis(Z/P=3.750/0.000,3.581/0.000 and 3.580/0.000).Conclusion The levels of TAC,cat,SOD,SIgA,IgG,IgA and IgM in saliva of caries children increased and the level of MDA decreased,which was closely related to the severity of caries.MDA,TAC and sIgA in saliva could be used as indicators to identify the severity of caries in children.
