腔内三维超声在宫腔疾病诊断中的应用价值

摘    要:目的:探讨经阴道腔内三维超声(3D-TVS)自由解剖成像(Omniview)技术在诊断宫腔疾病中的应用价值。方法:选择2018年6月-2020年6月入院手术的80例宫腔疾病患者,所有患者术前均行二维超声(2D-TVS)与3D-TVS检查,并在3D-TVS基础上利用Omniview技术容积数据进行三维重建成像,将临床病理检查的结果作为金标准,评价腔内三维超声在宫腔疾病中的诊断效果。结果:80例患者手术病理结果为:子宫内膜息肉18例、黏膜下子宫肌瘤19例、子宫内膜增生26例、宫腔粘连15例、子宫内膜癌2例。两种检查方法对子宫内膜息肉、子宫内膜增生、子宫内膜癌诊断的灵敏度和特异度比较差异无统计学意义(P>0.05)。3D-TVS Omniview对黏膜下子宫肌瘤、宫腔粘连诊断灵敏度显著高于2D-TVS(P<0.05),但特异度比较差异无统计学意义(P>0.05)。3D-TVS Omniview对宫腔疾病诊断的约登指数均高于2D-TVS,其技术效果和真实性高于2D-TVS。3D-TVS Omniview技术对黏膜下子宫肌瘤和宫腔粘连分型诊断的灵敏度要显著高于2D-TVS(P<0.05)。结论:3D-TVS Omniview技术诊断宫腔疾病具有较高灵敏度和特异度,在黏膜下子宫肌瘤及宫腔粘连的诊断方面优于2D-TVS,可作为宫腔疾病检查的首选方式。

Abstract:Objective:To explore the application value of transvaginal three-dimensional ultrasound(3D-TVS)free anatomical imaging(Omniview)technology in the diagnosis of uterine cavity diseases.Methods:Eighty cases of patients with uterine diseases who underwent surgery in hospital from June 2018 to June 2020 were selected as the research subjects.All patients underwent preoperative two-dimensional ultrasound(2D-TVS)and 3D-TVS examinations,and three-dimensional reconstruction imaging was performed using Omniview volume data.The results of clinic opathological examination were used as the gold standard to evaluate the diagnostic effect of intrauterine three-dimen sional ultrasound in uterine diseases.Results:The surgical and pathological results of 80 patients were as follows:18 cases of endometrial polyps,19 cases of submucosal uterine fibroids,26 cases of endometrial hyperplasia,15 cases of intrauterine adhesions,and 2 cases of endometrial cancer.There was no significant difference in the sensitivity and specificity of the two inspection methods in the diagnosis of endometrial polyps,endometrial hyperplasia,and endometrial cancer(P>0.05).The sensitivity of 3D-TVS Omniview was significantly higher than that of 2D-TVS in submucosal hysteromyoma and intrauterine adhesion(P<0.05),and there was no significant difference in the specificity of the two methods(P>0.05).The jordan index of 3D-TVS Omniview technology is higher than that of 2D-TVS,and its technical effect and authenticity are higher than those of 2D-TVS.The sensitivity of 3D-TVS Omniview technology in the classification diagnosis of submucosal myoma and intrauterine adhesions was significantly higher than that of 2D-TVS(P<0.05).Conclusion:3D-TVS Omniview technology has high sensitivity and specificity in the diagnosis of intrauterine diseases,which is superior to 2D-TVS in the diagnosis of submucosal uterine fibroids and intrauterine adhesions,and can be used as the preferred method for the examination of intrauterine diseases.
