桃红四物汤对桡骨远端骨折患者腕关节功能及骨代谢的影响

目的:探讨桃红四物汤对桡骨远端骨折患者腕关节功能及骨代谢的影响。方法:选取2017年3月—2019年3月我院正骨科收治的120例桡骨远端骨折患者,按随机数字表法将其分为对照组(60例)和观察组(60例)。对照组采用手法复位夹板外固定进行治疗,观察组在此基础上给予桃红四物汤进行治疗,治疗8周。比较两组术后8周的临床疗效;对比两组术前、术后8周的血流流变学指标、骨代谢指标水平及腕关节功能和视觉模拟评分(VAS)评分;统计两组治疗期间的并发症发生率。结果:观察组总有效率为96.67%(58/60),高于对照组的80.00%(48/60)(P<0.05)。与术前相比,术后8周,两组腕关节功能评分均升高,且观察组高于对照组(P<0.05);两组VAS评分均降低,且观察组低于对照组(P<0.05);两组全血高切黏度、全血中切黏度、全血低切黏度水平均降低,且观察组低于对照组(P<0.05);两组血清碱性磷酸酶(ALP)、骨钙素(BGP)、护骨素(OPG)水平均升高,且观察组高于对照组(P<0.05)。观察组并发症发生率为5.00%,低于对照组的13.33%(P<0.05)。结论:桃红四物汤治疗桡骨远端骨折,可明显升高患者血清ALP、BGP、OPG水平,增强腕关节功能,并能减轻患者疼痛,降低其并发症发生率,疗效显著。

Effect of Taohong Siwu Decoction on Wrist Joint Function and Bone Metabolism in Patients with Distal Radius Fractures

Objective:To investigate the effect of Taohong Siwu Decoction on wrist joint function and bone metabolism in patients with distal radius fractures.Methods:A total of 120 patients with distal radius fractures were randomly divided into the control group(n=60)and the observation group(n=60).The control group was treated with manual reduction and splint external fixation;on which basis,the observation group was also treated with Taohong Siwu Decoction.The clinical efficacy was compared between the two groups after 8 weeks of the treatment.The levels of bone metabolism,hemorheology index and the scores of wrist function and visual analogue score(VAS)were compared before and after 8 weeks of the manipulation,and the incidence of complications was counted during the treatment in the two groups.Results:The total effective rate was 96.67%(58/60)in the observation group,which was significantly higher than 80.00%(48/60)in the control group(P<0.05).The scores of wrist joint function were increased after the treatment compared to those before the treatment in the two groups;of which the increases in the observation group were more significant than those in the control group(P<0.05).The scores of VAS were decreased after the treatment in the two groups;of which the decreases in the observation group were more significant(P<0.05).The whole blood high-shear viscosity,the whole blood medium-shear viscosity,and the whole blood low-shear viscosity decreased in both groups,of which the observation group was lower than the control group(P<0.05).The serum levels of alkaline phosphatase(ALP),bone glaprotein(BGP)and osteoprotegerin(OPG)were increased in the two groups,of which the improvements were more significant in the observation group(P<0.05).The incidence of complications was 5.00%in the observation group,which was significantly lower than 13.33%in the control group(P<0.05).Conclusion:Taohong Siwu Decoction combined with manual reduction and splint external fixation can significantly increase the serum levels of ALP,BGP and OPG in the treatment of distal radius fractures,it can enhance wrist joint function,alleviate patients’pain and reduce the incidence of complications with its significant effect.
