早发型重度子痫前期母儿预后

目的:探讨早发型重度子痫前期对围产期母婴预后的影响。方法:对本院64例早发型重度子痫前期和98例晚发型重度子痫前期患者进行回顾性分析。根据其发病孕周分成4组,即a组(孕周<28)、b组(28≤孕周<32)、c组(32≤孕周<34)、晚发型组(孕周≥34)。结果:早发型重度子痫前期患者并发症发生率显著高于晚发型重度子痫前期患者(P<0.05);新生儿窒息率和围产儿死亡率a组最高(P<0.05),b组次之(P<0.05),其他两组均较低且两组间差异无显著性(P>0.05);b组期待治疗时间明显长于其他各组(P<0.05);各组患者的分娩方式均以剖宫产为主。结论:早发型重度子痫前期严重影响母婴预后,在期待治疗过程中,应严密监护母胎情况,适时终止妊娠,终止妊娠的方式首选剖宫产。

Study on the maternal and neonatal prognosis with early-onset severe preeclampsia

Objective:To study the effect of early-onset severe preeclampsia on the maternal and neonatal prognosis.Methods:A retrospective analysis was carried out on 64 patients with early-onset severe preeclampsia and 98 patients with late-onset severe preeclampsia. They were divided into 4 groups according to pathogenic week of gestation: group a(<28 W),group b(28≤W <32), group c (32≤W<34) and late-onset severe preeclampsia group(W≥34).Results:The rates of pregnant complications of patients with early-onset severe preeclampsia were significantly higher than those of ones with late-onset severe preeclampsia(P<0.05); The neonatal asphyxia rate and perinatal infant mortality of group a were the highest(P<0.05), followed by those of group b (P<0.05); Those of the other two groups were lower, and there were no significant differences between the two groups(P>0.05);Expectant treatment time of group b was significantly longer than that of the other groups(P<0.05).Cesarean section was a main method of pregnancy termination for all groups.Conclusion:The early-onset severe preeclampsia badly influences maternal and neonatal prognosis.During the expectant treatment , the situation of maternity and fetus should be carefully monitored. The pregnancy termination should be in good timing ,and cesarean section is the first choice.
