重度氟斑牙直接贴面黏接托槽降低托槽脱落率

摘    要:目的 :探讨正畸重度氟斑牙采用光固化复合树脂直接贴面黏接托槽的有效性 ,降低临床托槽的脱落率 .方法 :选择 1 7例 (共 32 4颗 )需用固定矫治器进行正畸治疗的重度氟斑牙患者 ,按氟斑牙釉质表面处理方法的不同 ,随机分为氟斑牙直接黏接托槽组、氟斑牙表面磨除黏接托槽组、氟斑牙直接贴面黏接托槽组 .每组牙经酸蚀和釉质表面处理后 ,每一牙面均黏接方丝弓托槽 ,观察托槽的脱落率 .结果 :氟斑牙直接黏接托槽组 ,氟斑牙表面磨除黏接托槽组 ,氟斑牙直接贴面黏接托槽组 4 ,8和 1 2wk总脱落率分别为 74 .0 %、2 5 .9%和 1 .7% .经 χ2 检验 ,3个黏接组间托槽的脱落率有明显的差异 (P <0 .0 1 ) .氟斑牙直接贴面黏接托槽组的疗效最佳 ,且氟斑牙直接贴面黏接托槽组效果好于氟斑牙表面磨除黏接托槽组 ,氟斑牙表面磨除黏接托槽组效果好于氟斑牙直接黏接托槽组 .结论 :光固化复合树脂直接贴面黏接托槽可有效降低氟斑牙正畸托槽的脱落率 ,是临床重度氟斑牙固定正畸矫治中的一种美观有效的新方法 .

Clinical study of reducing losing ratio of brackets bonding to heavy fluorotic teeth
Abstract:AIM: To investigate the effects of bracket bonding to heavy fluorotic teeth with the light curing composite and to reduce losing ratio of the brackets. METHODS: 324 heavy fluorotic teeth were chosen in 17 patients who needed fixed appliance for orthodontic treatment. According to different treating methods of fluorotic teeth enamel surface, the teeth were divided into 3 groups randomly: direct bonding to fluorotic teeth surface, bonding to polished fluorotic teeth surface, and bonding to veneers of fluorotic teeth. The losing ratio of the brackets were observed after 4, 8 and 12 weeks. RESULTS: The losing ratio of the 3 groups was 74.0%, 25.9% and 1.7% after 4, 8 and 12 weeks of bonding, respectively. There was significant difference in the losing ratio of the brackets among the 3 groups( P <0.01). The effects of bonding to veneers of fluorotic teeth surface were much better than those in the other two groups. Bonding to polished fluorotic teeth surface was better than direct bonding to fluorotic teeth surface. CONCLUSION: The losing ratio of the brackets bonding of the heavy fluorotic teeth with the light curing composite can be reduced significantly. It is an effective method for heavy fluorotic teeth, which can be used in fixed multibrackets appliance.
